Treatment Emergent Central Apnea - mikesj - 08-23-2024

I'm looking for assistance in really improving my AHI. I have tried Cervical Collars which offered me no substantial improvement to my numbers, but recently over the last week I did go to a wedge pillow with a nearly 30 degree incline 25x25x12.  This has shown the biggest improvement in my numbers, but i'd still like to see if there are any ways to improve them further.

Additionally - is there any thoughts to will surgery possibly help with the positional apnea that seems to be causing my issues.  I'm not actually sure if my machine is doing anything as the decrease went from 12AHI just by changing to the wedge. Trying the Cervical Collar with the wedge increases my numbers.

My research mentions ENTs as the specialist to see for stuff like surgery and such. I am hopefully getting a referral to one soon. 
If you aren't already seeing an ENT it may be helpful to do so.

sorry you did not get any takers. sometimes things kind of get lost especially if traffic is high. It was good for you to refresh this so it puts post up front again. hope you do get some responses this time.

I suggest setting your minimum EPEP 11, maximum IPAP 15, and PS 3.2 to start.

Surgery will not fix Positional Apnea. It's a rather simple action, keeping your chin from tucking and kinking the airway. Positional Apnea is an external, physical force restricting the airway.

A big wedge incline might make this worse. If it can support upper body, can mean less of a negative effect though.

Sleeping on a wedge pillow tends to worsen positional apnea, despite your one better night.  If your neck collar is the right height, it should be all you need for positional apnea.

Your leak rate looks quite high to me. I know you say your issues are positional, but that could be causing you issues as well.

Thank you I'll give that a try and see how it impacts.

I really struggle with not rolling around in my sleep which the nasal pillow pulls even a little from me when I move cause high leak rates. I might need a new style of mask where the tube isn't on front since I move so much

@debora I tried a handful of different collars with no improvement, but each day I actually stay on the wedge and sleep my numbers are lower. Albeit after a week I can't stand sleeping on it as I tend to sleep very few consecutive hours now. 

Do you have a suggestion on how to better find a good fitting neck collar?
