I'm a first time poster but longtime CPAP user (since 2008).  Last November I replaced my ResMed AirSense 10 with a Card-to-Cloud version and transferred the settings from the old to the new.  Everything was fine ... until it wasn't.  It took months but I finally realized my sleep wasn't a "good" as in the past.  I started to check my AHI numbers and was alarmed to see how high they were.  I downloaded OSCAR and reviewed the data.

I need to adjust my machine settings but I don't understand what the data is telling me to do.

Thanks in advance for any help or advise on what machine settings are needed or to try.  OSCAR data file attached.

You should increase your EPAP to 10 cm at least. Also, turn your RAMP off.

A further increase might be needed. But first, you should take care of your positional apnea because the clustering of events can indicate that. Use a thinner pillow to avoid chin tucking, and try to sleep on your side.

Thanks for the quick response. I increased the pressure to 10 cm and turned the ramp off. My CPAP pillow is about 3 inches tall but I have a travel CPAP pillow that is a bit thinner if needed. I always start sleeping on my side and always wake up on my back. I ordered a Caldera Releaf Neck Rest to see if that helps with the chin tucking.

I'm hoping for a better night tonight.

Looks like you reduced minimum pressure. This is an Autoset, so your current settings are 4-20 with EPR 3. Looks like you need a minimum pressure of 10.0, which will result in pressure of 10/7 (inhale/exhale). Your 95% pressure is nearly 13, so we may need a bit more pressure, but start at minimum 10, and we can see how it goes.

I agree with Sleeprider that the pressure setting of 10 min. would have been my pick, with the EPAP of 7 to provide exhale support pressure. Going higher than that might be necessary, but I would rather look at supporting your unobstructed breathing at time when the flow limitations are low to non-existent, and counting on positioning to resolve excursions like what happened at 00:35:00.
