help please, not sure what to do now - jasonj - 10-28-2024 I have been using the cpap for a little over a month now, and thanks to an earlier post here i believe i have my pressure settings pretty much dialed in. feel free to tell me how wrong i am though. the problem is the metric butt load of central apneas. i did a 2 night sleep study to start all this and the first night had 190 obstructive events, no centrals, the 2nd night i had 180 events, and 2 centrals, for reference. whenever i wake, or "doze" half awake / half asleep, it shows as constant central apneas. you can look at the oscar data and easily tell when i wake, thick bands of centrals. if i lower the pressure, i get obstructives. if i lower the EPR, i get butt loads of hypopneas. ive attached last nights data, as well as a couple of earlier ones. not sure what, if anything, i need to do now. do i not worry about the centrals given that im basically awake when they are happening? is this treatment induced and should resolve eventually? any help/thoughts would be appreciated. RE: help please, not sure what to do now - G. Szabo - 10-28-2024 I suggest turning EPR off for now. Also, post some 3-minute windows of the flow rates of your CAs; perhaps they are misclassified. RE: help please, not sure what to do now - jasonj - 10-28-2024 turning EPR off spikes my hypopneas big time. case in point attached. also attached a 3 min flow rate window RE: help please, not sure what to do now - G. Szabo - 10-28-2024 I suggest setting pressure min/max 12/14 cm and EPR=3. Please post the outcome. RE: help please, not sure what to do now - jasonj - 10-29-2024 here's last nights. still centrals out the wazoo. RE: help please, not sure what to do now - G. Szabo - 10-29-2024 You had a smooth pressure ride with a few adjustments but still have some Hs. Hence, it is not advised to reduce EPR despite the Cs. The centrals should disappear within about two months. If not, and you still have Hs, then you need a different device. Also, try to reduce the leaks because they can trigger Cs. RE: help please, not sure what to do now - jasonj - 10-29-2024 thanks for your help! RE: help please, not sure what to do now - Expat31 - 10-29-2024 Hi, it would be useful to have full details of your sleep study with such high obstructive numbers, 180-190 events. There seems to be something really going on here. Can you can post this sleep study report, with your personal identification redacted. We may spot something. |
