"Bilevel is the CPAP everyone should have. Resmed made their CPAP a bilevel, but limited to 3-cm of pressure support (EPR). So why doesn't everyone get bilevel?     Is it known or generally considered to be true—all other things being equal and ideal—that within the effective therapy range of the Resmed AirSense Autoset's EPR, the Autoset and the Resmed AirCurve Vauto would sense and record identical sleep metrics? It seems the firmware difference SR cites must simply prevent Autoset pressure support greater than 3.0-cmH20.

(In a bygone day, we once called in IBM to increase speed of our—my employers'—tractor feed printer for their then-punchcard driven IBM System 3 computer. One small part was removed from the printer. The lease rate was immediately bumped up, but at least there was an increased wear factor to justify some increase.)

While the hardware of the two instruments can be identical, my understanding is that we do not know what alterations they made in the AirCureve software besides allowing higher pressure support.
To find out, we should run the two instruments with the same person at EPR3/PS3 settings and compare the AHI and FL values.

From my perspective, the AutoSet and EPR 3 versus a VAuto with PS 3 can be similar yet different. And this is per the individual person using it.

Similar: effectively it's PS 3, a differential between EPAP and IPAP

Difference: the VAuto has Trigger, Cycle, Ti Min/Max which can/will affect therapy and feel, and Max IPAP 25

Although briefly, I've tried both a 10 series AutoSet a few years ago, and recently the 11 series VAuto. For my needs, the VAuto would have been better. Delivery felt better/different than AutoSet.

Besides the algorithm, they are supposed to be the same, at the least very similar in the same series
