Need help concerning BiPAP therapy - Marlone - 11-26-2024

Hello everyone,

I've been experimenting with BiPAP for about a month now. My average breath doesn't look too terrible more in OSCAR (see screenshot 1), however, there are still a lot weird breaths every 10 mins or so (screenshot 2) which I suspect to be arousals. I also still feel very symptomatic unfortunately.

My settings are

Aircurve10 ST
IPAP 11
EPAP 4
Backup Rate 10
TiMax 4s
TiMin 1s
Rise time 900ms
Trigger High
Cycle Med
Full face mask.

Any suggestions what to change? Could ASV be an option? I'm happy for every help about this.

EPAP 4 IPAP 11 is rather low pressure, but the resulting PS is high at 7. This can contribute to your off patterns.

Also do you know why you're on an ST? This isn't your typical CPAP or bilevel for treating just Apnea. ST is a spontaneous timed device for lung conditions like COPD that needs a timed breath to make sure the user wouldn't miss breaths. (Simplified explanation)

So let's start here, why do you have a specialized bilevel ST? I do see you're not using timed mode, so it's not being timed with this S mode you're on. However this is very basic, static pressure, where a VAuto would be able to give pressure ranges that can edit itself overnight as needs arise.

Might be that there's 2 big things against you: wrong machine and poor pressures availability.

I got the ST from my sleep doc after my request of wanting to try Bilevel. After some research done I also notice that it doesn't really fit my requirements. I'm on my way to get a Dreamstation DSX900.

I think you would be a lot happier with a ResMed AirCurve 10 Vauto.  Most of us here strongly prefer the ResMed machines to the Philips machines.  Many of us switched from Dreamstations to ResMed and sleep much better with the ResMeds.  I know my husband and I do.
