HR spikes significant? Why do I still feel awful? - UsuallyTired - 12-21-2024

Hi all, 
I've been trying to treat my apnea for over a year, and I'm still struggling with debilitating fatigue. 

I've never been able to tolerate settings about 6 without oral venting (even with taping), but recently that's changed. I don't know why. Perhaps it's b/c I've just completed the palatal expansion phase of my MARPE, and it's improved my tongue position. 

I have been wearing an O2Ring lately, and here are last night's results. I have felt miserable today and was barely able to function. 

Some questions: 
  • Are the HR spikes significant? They often don't line up with flow limitations. 
  • What is driving increased CAs (they've increased as I increase pressure and/or EPR)? 
  • Could my supplemental O2 @ 1lpm be worsening any of these problems? 
Anyone familiar with Patrick McKeowan's work, particularly his recommendation that oversensitivity to CO2 can worsen apnea and numerous other problems? Would it be worthwhile to try his exercises? 

Notes: 
  • Normally I have very little leak. I think my cushion is old, so tonight I'm gonna try another. 
  • My Tidal Volume and Minute Vent are always on the low side, and I don't know why. My Resp Rate is always around 17 or 18. 
  • Not last night, but occasionally I'll wake up feeling like I can't get enough air from my CPAP and am suffocating, and I have to take it off or breathe through my mouth for a while.
