Aircurve 10 ASV Auto not sleeping very tired - RonApneaBoard - 12-31-2024

In 2019 I had a sleep study and found out I had Central Apneas and Hypopneas. I was given the original settings of IPAP 8/35, EPAP 5/15 and PS 3/20. in the following 6 years I increased the EPAP min by 1 cm and as of 12-29 the settings are IPAP 14/30, EPAP 10/15 and PS 4/15. I have had excellent results for 6 years (less than 1 AHI). When I saw the AHI exceed 1 over a period of time, I would increase the EPAP min and that would resolve the problem. As of a week ago I have not been able to sleep much at night and this has caused me to be in a very foggy state and very tired. I am going to attach Oscar results for the evening of 12-29 which will show me frequently getting up not being able to sleep. Last night in the middle of the night I decided to increase the PS min from 4 cm to 6 cm and that seemed to help me a lot. I do not have that data I may have had the card in the no-write position. Would you agree that this change is the way to go. I wanted to increase the PS to 7 cm but the machine will not let me make the change. I did decrease the EPAP min from 10 cm to 9 cm but did not like the feeling so I returned it to 10 cm.

We'll need the OSCAR chart with the left panel included. OSCAR should be an F12 screenshot capture if your computer does it.

As a former ASV user myself, it sounds like your EPAP and PS Min might be too high. My own settings for the 2 years I had a ResMed AirCurve 10 ASV were:

ASV Auto mode
EPAP 7-13
PS 3-15 default
IPAP 10-28

Your ResMed ASV PS range needs minimum 5 between Min and Max. Example lowest PS is 0-5.

Thanks so much. I got it figured out now.

What settings have you tried, for how long, and the results? Thumbnail sketch is fine for now.

It appears you had edited EPAP min up if you had 1 AHI, sound correct? Under 5 AHI is treated, yet we can do better. However you don't need zero AHI. The goal in my opinionis lower than 5 AHI and well rested and comfortable.

I'll suggest editing EPAP Min to 7 or so for a night. Keep PS as is for now. I'm thinking your overall minimum EPAP is a bit high.

Dave -
I did send new attachments with sidebars as you requested.
I have to leave for 3 - 4 hours soon.
When things settle I will send in a donation.
So just to make sure. You would like me to try EPAP min at 7 and leave PS at 4.
IPAP 11/30
EPAP 7/15
PS    4/15
Thanks for all the good info.

Yes that's right. Do take note your Max IPAP might mathematically go above 25, but the ASV physically caps Max IPAP to 25.

Smile Dave it worked. Best sleep I have had in a long time. I slept 8 1/2 hours last night without waking up or getting up even once and the AHI was 0.0. You solved my problem many thanks. If there is anything else I should do like remove attachments or delete the thread let me know and explain how to do it.
