ASV Settings
I have had an ResMed AirCurve ASV for about a month now. The recent settings work pretty well. I had severe sleep apnea and used to have an AirSense 10 AutoSet but had mixed apnea and complex apnea and now have an ASV. My recent settings are EPAPmin=7, EPAPmax=15, PSmin=2.4, PSmax=18. This is the result, and it is a significant improvement before probably due to lowering PSmin from 4 to 2.4. I plan to change new settings tonight to EPAPmin to 6 and PSmin to 2.6, and PSmax to 15. Does this make sense to you? I will post the results in the morning. I hope I'm getting close to the optimum settings. What are your suggestions?

All of the residual events are obstructive and appear to be positional. Watch out for chin-tucking. https://www.apneaboard.com/wiki/index.ph...onal_Apnea

I tried eliminating my wedge pillow and just using a thin pillow. I tried to keep from tucking my neck but did not have any improvement.

I eliminated the wedge pillow and only had a thin pillow. It did not seem to help, although I tried to avoid tucking my chin. I will go back to using the wedge pillow and avoid tucking my chin tonight.

my understanding is ASVs are highly sophisticated and basically a turnkey product. If obstructions persist, the only troubleshooting step is increasing EPAP until they subside.

Jwest, learn about the positional apnea paradox. These types of obstructions do not respond to higher pressure, and the apnea in the example above were not eliminated, even above 13 cm EPAP. We could increase EPAP to the point that the ASV would no longer function for central apnea, and would be very uncomfortable and still not fully resolve the obstruction arising from positional obstruction.

ahh ok yeah, that makes sense. I yield. It does look like even at extremely high pressure they did not resolve. Thanks for the explanation.

Thanks, I agree about EPAP settings. I have EPAP min set to 6 and it usually goes up as necessary since the ASV is set to ASV Auto mode. I will get a soft cervical collar to try out. If anyone has a suggestion, let me know. Thanks in advance!

I had the best night so far on my ASV. AHI was 1.19, mainly because I concentrated on only sleeping on my sides. I had 0 unclassified apneas, only hypopneas. I usually have mainly obstructive apneas when sleeping on my back, so the AHI worked perfectly to eliminate the centrals when on my side. I know my sleeping positions because I recorded the night with my video camera, and correlated the timeline with OSCAR. ASV is awesome!

That's great! you have figured out what works and found a way to minimize the positional events. Hard to beat that.
