The common cause of Centrals, not the only one, is an incorrect level (not enough) of pCO2 in the blood. The chemoreceptors fail to send the signal to breathe when there is not enough CO2 in the blood. Some individuals are much more sensitive to this than others. Anything that affects breathing rate and/or volume has the potential to impact central apneas, not always in a positive way.

So not just EPAP, but everything that is associated with breathing.

Hi folks, checking in again. I've had a really rough 10 days since my last check-in. I experienced mild aerophagia most nights on EPAP-IPAP range of 4-9 with PS 3. I met with a pulmonary sleep specialist that I'd been waiting to see for over 5 months. He is going to petition my insurance provider again to give me a polysomnography. He also took a look at my data and adjusted my settings to EPAP-IPAP range of 4-10 with PS 2. It has only been two nights, but with this change I no longer experience aerophagia. My RDI has also gone down and ranges from 0.22-1.26. 

However, I have the worst brain fog I've experienced in months. My FLs are up to 0.12-0.31 and I'm waking up between 4-9 times. My FR also has multiple "flattened curve" sections similar to when I was on the AirSense. I've been experiencing a lot more leaks lately with the P10, even with Lanolin. I just purchased a Bleep and am hoping that works better for me. 

I used a Dreem 2 last night for the first time and it showed 9 wake-ups. Perhaps wearing a new device contributed to the wakeups because I'm not used to it yet. I also noticed that I tossed and turned basically every 5-10 minutes all night long. Will post more results from the Dreem in a few days.

Should I stay the course with these new settings for another week, or change anything?

While I don't have settings change advice, I am definitely interested in your subjective experience with the Bleep mask once you get it. Great to hear aerophagia was improved. When it is bad, it is genuinely awful. Best of luck!

Hopefully your insurance company will approve that sleep study.

My guess is the reduced PS solved your aerophagia, but increased your RERAs. Not sure what to do about that.

Maybe you could try incrementally raising PS again by 0.2 every few nights and see what happens?

Oh, on Bleep, you'll probably find it a bit odd the first few nights, but if you follow the directions, you should be able to get zero (or very close to zero) leaks, and no more need for lanolin.

Please, I would like to see some 10min-windows from your TV contrasts, to see if we could have a better idea on what is provoking your arousal/awakening/wake-ups. Hopefully, these would be respiratory-driven events.

Another variable to keep in mind that can lead to inaccurate results: signal quality. During my 7 night assessment, I had a couple of nights with less-than-great data. I guess that's one reason they do the assessment over 7 nights.

You can always ask Dreem support about that?
