As I search my optimal pressure, should I increase by 1 cmH2O per week at most?
Increasing by 1 cmH2O per week at most seems to be the standard recommendation (once I saw someone recommending every 2 weeks). Yet, this video https://www.youtube.com/watch?v=-IIZdru6vus shows that technicians in lab sleep studies increase by 1 cmH2O every 5 minutes.

Seems like a contradiction to me. Can anyone clarify please?

Hello!

I'm very new to ApneaBoard and OSA in general as I was only diagnosed recently, however, I believe I understand why this difference exists.
It's my understanding that a technician can titrate event-by-event, watching for obstructive apneas, meanwhile when titrating from home, we want to minimize the chance of going too high and causing discomfort/aerophagia and cannot see the results of changes until the day after.

That being said, I haven't browsed enough threads yet to be able to say definitively that this is true and/or that you should only increase once per week. I personally think giving it multiple nights seems logical to minimize the chances of outlier nights, but again, I'm very new, so take everything I've said with a grain of salt. It'd probably be best for you to wait for someone who's more knowledgeable to come in and explain this to you, but I'm leaving this reply anyway just in case nobody does.

The reason you can't adjust as fast is because you aren't awake watching your data in realtime, or if you are it's not good data  Grin
Breathe through your nose
Reduce sugar and processed food
Soft collar and tape

Sleep-well

I disagree.  If you need to make a big change in your starting pressure, I think you should do it all at once. Then, if you find the adjustment difficult, back it off by 1cm or 2 cm at a time. Why drag out making a beneficial change?

We will often observe results in Oscar and make relatively large corrections early in therapy if needed; however, as someone closes in on optimization, we tend to suggest smaller changes or even less than 1-cm. We have a pretty good idea how to address problems, and we know them when we see them. Post a chart, and we can take this out of the realm of theoretical.

so you have a chart to share, i'm currently getting OA/Hyponia concurrent with EPAP ceilings, at 7-11, full time, 3 , over a few nights,  been at these new full, 3  for 4 days, was at ramp only, 1  , and 6-11 , was thinking to wait a full time days of Oscar data, then increase top pressure to 11.5 ? 

I could share a chart again, but does That makes sense to do?

Thanks all! Unfortunately I have no sleep/night-time data to share - only (presumably useless) ~6h daytime data spread across two days..
