Still waking up unrefreshed, chest tiredness/tightness, knees bent up
I am still struggling some nights to get restful sleep. My BI-LEVEL and ResMed Oral Dental Appliance (Narval) is only helping so much. I lay on my back most of the night, due to pain in my legs if I lay on my side. Last night, my webcam recorded I had my knees bent up for about 30min. It showed me removing my bed covers, and then bending my knees up. I have no memory of doing that in my sleep. If I were to send these Sleepyhead charts to my Sleep todcotr, he would say "all is fine". I woke up feeling tiredness/tightness in my chest. I've had my heart tested throughly and it is fine for now, but I keep feeling this tiredness in mornings, whenever my machine seems to have ramped up for hours at night for no reason.

I'm not sure if I'm still struggling to breathe at night or not? Should I get yet another sleep study done or opinion from a doctor that does UARS testing with a PES device?  Why is this machine ramping up so much, if I don't have any obstructions during a period of time?

How about seeing what happens with a maximum pressure of 14.0? We can see if there are consequences or you feel better.

Good advice. But unfortunately already tried that. My doctor originally had it on Max 14.0 after my last sleep study. Some nights it was pegging out at 14.0 and when that happened I started having even weirder breathing issue. My flow rate would show I would take a deep breath, then it would get really shallow, and then take a deep breath again. Looked like sideways cones on the graph. It made my heart go into Ventricular Bigeminy due to my throat closing off and choking myself multiple times in a row. But that was before I used a cervical collar I think. 

I will have to look at my history and notes again. But I'm getting tired of spending all this damn time with this machine and my sleep issue. It's consuming my life. 

I have an appointment with my sleep doctor next week. But it's worthless bringing up any issue like this to him. My AHI < 1 and he will say I'm doing great. But obviously I'm still having trouble breathing due to the way my throat and anatomy is made. I have narrow mouth, big tongue, etc. 

I'm not going to waste another year of my life feeling this way! I'm going to a surgeon and another ENT to talk about MMA and other options that can open my airway.

Just out of curiosity, what webcam are you using? I was thinking of setting one up to get a better sense of the ways I might be moving at certain times in the night.

You could try raising IPAP and EPAP min by 1cm h2o, so min EPAP 5 and IPAP 9, to hopefully deal with those remaining obstructive and flow limitation components. Also the feeling of a bit more air may feel more comforting. For me, at 8/4 I'd have to check if my machine is even on these days!

Your statistics besides AHI look good. RR, tidal volume etc., all looks normal.

Are you knowingly waking up at all in the night? How is your caffeine usage? Any medications by chance?
