Not sure where to go with these results....
Hello,
After growing up as a 'snorer', I was finally diagnosed with severe OSA just after turning 40. Identified as someone with a slightly deviated septum I persevered with a (ruddy expensive) dental device for a couple of years before adding CPAP. Almost 6 years in, and I'm still trying to get things 'tuned up'. I started out using a full face mask but found that a nasal mask is a lot more comfortable, and have had some pretty decent nights' sleep - so I do still remember what they are supposed to feel like!
With Resmed's 'CPAP report for Dummies' app it didn't take long to find Oscar (n'ee SleepyHead), which I have found enlightening. However, despite the Technicians making a lot of small adjustments over time, my sleep 'signature' remains the same. I usually doze off quickly, and the Airsense 10 cranks the pressure up to the max setting. The most recent Tech adjustments have increased the starting pressure again and also raised the ramp so I get to 20 that little bit sooner. Despite this, my AHI graph always reaches the mid to high-teens in the first hour of sleep, at which point I regularly wake, pop to the loo, and get set up again. Come morning I am typically blown up like a balloon and spend some minutes 'depressurising'. As an experiment to try and defeat the eye-level air leaks that plague me when turning over in bed I recently spent a week using an F30 'hybrid' mask. After quite a lot of fiddling about I managed to achieve a huge improvement in mask sealing, yet despite setting the machine back to Full-Face, the reported apnea numbers were huge and I felt dozy in the morning. I have read about people having issues if they forget to update the Mask Type setting, so the machine miscaculates the mask volume etc., but mine was set correctly.
Please see the couple of screenshots below, firstly of a typical night with my usual N20, and secondly of a trial with the F30 mask:
Looking at these, and rarely getting a nightly AHI below 7 (okay, so my first sleep study gave me 37), I'm not sure what to bring up with the Techs. Unless I'm somehow reacting negatively to the high pressure, my apneas continue to climb when the Airsense 10 is already at its pressure limit. As this is my first post, I could of course also be talking nonsense.......
Thanks in Advance,
James
