Central Sleeping Apnea treatment
Hi all,

After performing a sleeping test, I was diagnosed with CSA.  My doctor has prescribed CPAP 7cmH2o, epr 3 for the treatment.
Any members here have also been prescribed with similar CPAP setting to handle CSA?

Typically with central sleep apnea someone would be prescribed an ASV. CPAP really isn't able to help much with CSA.

How long have you been using the CPAP and do you have any OSCAR charts to post?

Would you be able to provide a copy of your sleep study? If you had true CSA as jcp519 noted you would have been given an ASV. You most likely had CA’s during the study, which a CPAP, APAP, or bilevel, would quite possibly be able to handle. If you do have it redact your personal data before posting.

Once we see the sleep study we can offer more sound advice. Without it we can still help, but it is a bit of a trial trying things willy-nilly to find where your therapy stands.

If you do not have it you can try one of the standard setups we use.

Min pressure 7
Max pressure 15
EPR 3 full-time
No ramp

After a few nights of that we can help tweak things further.

I attached my sleeping test result below. My doctor has prescribed to use fixed pressure (no auto) 7cmH2o, epr 3 as treatment.

Do you have a CPAP machine currently? If so what kind of machine? Having OSCAR data would help us figure out settings to try.

If you do not have a machine as yet, now is the time to ask the sleep doc or tech about the CA's and what machine they think will help. If it is not an ASV, ask why not if they are true centrals.

Dear Peace Love,
The reasons for not using ASV is the cost of the machine.  My doctor is not sure if I can adapt to use a respiratory machine and would like to start with CPAP. Otherwise it would be a waste of money.

In view of my sleeping data, is there any member here also using CPAP to treat similar CSA pattern?

I think it's part of 'jump through hoops' requirements for insurance to cover the costs. I had to try, and fail, 3 machines before they put me on ASV. It also meant I had to have 4 in-lab sleep studies (not including 2 home sleep studies) before I would qualify for an ASV. But, not once was it explained to me. But all that depends on who is paying for it.

Yep, the above posts are true. Eventually you're likely to need and be issued an ASV to treat Central Apnea. The best would be the ResMed AirCurve 10 ASV in that case. However it is possible a CPAP/AutoSet or BPAP/Bilevel/VAuto might allow treating Obstructive based events and avoid some Central based events. The difference is very likely with your sleep study results that you might not be able to avoid enough Centrals with another machine to be truly satisfied with results and comfort than with the ASV.

Another thing to note is regular CPAP and BPAP will likely be somewhat to produce more comfortable therapy pressure delivery, but this does not mean we cannot coach the decent setup of an ASV that treats well with comfort. It may be you need a bit more diligence and patience to do trial and error, likely applies to any machine you'll get due to the CA. Conversely, the same patience and trial end error settings will likely be required with CPAP and BPAP without backup. ASV has a backup rate involved in CA treatment.

Note that for the Centrals I think you have shown in the test the ASV will treat them, and with CPAP and BPAP without backup rate you'd attempt to avoid Centrals.

I don't know how your insurance and medical systems work, so you'll need to decide what direction. I think though you'll eventually need ASV to treat Centrals. Your two choices are get ASV at the beginning or try CPAP, BPAP without backup rate, maybe an ST, then ASV. The ST would be more expensive than the CPAP and BPAP without backup rate but somewhat better at treating Centrals, but not near the ASV level, while a similar price. This is based on what we deal with in the US.

Thanks for the reply.

In my case, I don't have any insurance to cover the machine.  So no matter which machine, I have to pay on my own.

Regarding my sleeping test result, you can see that out of the 15 AHI, 10.9 are detected as CSA.  Is this typcial CSA pattern?

Is the CSA serious?

Frankly speaking, currently I can sleep quite well without any CPAP machine and not feeling tired in the daytime.  I am struggling if I should use a CPAP machine.

In fact I have tried a CPAP for 2 weeks.  The sleeping experience is not good even (worse than sleeping without a CPAP machine) though the AHI index (around 5)  so reported is better than without a CPAP machine.

Any member here has also been diagnosed with similar CSA issue? Please share.
