I will give up APAP treatment then.
I couldn't sleep with 8/EPR3. I changed to 8/EPR2 i slept better and feel better but the data did not looked better!

Pearl you might give this man a try at looking over your OSCAR data with you. He is a Registered Polysomnographic Technologist and might be able to help you find your way to an ASV if you really want to get one. Just an idea.

I will now open to other treatment options to see whether my sleep need to improve. eg. Traditional Chinese Medication - Acupuncture to treat my Central. I didn't asked for zero index to sleep with a APAP machine but looking at the statistics with various settings that i went through plus i don't feel significant different with and without the machine hence i decided to quit for now. I don't need to have that tube all over me for now hahaha. Having say that, i do enjoy the cool air at my nostril though with low pressure low EPR hahaha Let's see how it goes, i will be back to this forum to read. I still have OSA with me not that it is going away. When i get older, i will likely need to sleep with the machine again for my OSA. Till then, take care!

Keep us updated on how it works out with the Traditional Chinese Medication/Acupuncture. Who knows...if it works you might be able to get other people off the hose.

I wouldn't expect you to feel significantly better with this machine. Your two sleep studies showed untreated AHI of 13 and 21 and both had at least some central apnea (can't see original study anymore but I see comments saying it was predominantly central/mixed). Your results on PAP have not been much better usually ranging from 10-20 (although mostly central on PAP compared to mostly obstructive without). The attempted treatment is an obvious failure and that is how your doctor should be treating it as well, not turning a blind eye to central apnea in two sleep studies and every night of PAP data...
I am not opposed to you quitting since the reason you are on cpap is a bit odd anyways but if you do end up noticing you are sleeping worse than at least you know what you need to do to try and get adequate treatment (ASV). The option that would make the most sense is to convince doctor to try and treat your central apnea or to find another doctor that will. A titration study would prove to them that this machine doesn't treat your central apnea if that is what it takes to convince them. Other option is to find a way to purchase an ASV yourself, unfortunately they are overpriced now thanks to CPAP shortage. Edit: Oh and I can't remember your age or if it was mentioned but central apnea is most commonly associated with medications (like opioids), medical conditions or older age. If young and there are no reasonable causes then looking into it with a doctor (MRI etc) might be worthwhile.

On the April 14th download your corrected AHI is 0.0. The obstructive events are actually centrals misread by FOT. All the centrals are post-arousal or sleep onset therefore should not be counted nor treated. While some breaths might have the appearance of flow limitation they are probably a result of cardioballistic contamination. ASV will not help and perhaps even hurt because the "centrals" are normal phenomena and need to be left alone. The recommendation of mandibular advancement is irresponsible. Your sleep quality is horrible. Horrible horrible horrible. Send me my buck.

Please do not get upset because i quit pap treatment! I am sad because of the way the discussion had evolved.
I honour each and every individual that contributed their time to my treatment and i strongly feel each contributor deserve to be respected. I do expect there are differences in each individual's opinions. That is something i will need to make my own judgement. However, i do encourage all contributors to discuss amicably together to find the best solution for a patients and people like me that "knew" nothing about the machine (i do know alittle now hahaha at least i know how to see the leak graph - mask leak or mouth leak in Oscar hahaha) and consulted 3 doctors but still no solution. Guys, Chills please! |
