Coyote, what is the limitation or restraining factor in 'trying to get it fitted'? Is it too tight, too high, too big...?

All of your charts are of the same day. Have you tried the soft cervical collar yet, as this looks to be a positional sleep issue.
http://www.apneaboard.com/wiki/index.php...cal_Collar Also, I believe you need a higher pressure. Why are you on fixed pressure? Why not use the auto feature, set minimum to 11 and max to 18cm. I am combining your two threads to keep your therapy questions in one place.
