Higher AHI Readings on AirSense 11 than AirSense 10
I have been using an AirSense 10 CPAP, Pressure 9 for the past seven years with excellent results. AHI events have consistently averaged below 2, and leakage usually at or near zero. This system was recently replaced with a new AirSense 11 with the same settings. My AHI event readings on the AirSense 11 have consistently been much higher, averaging around 8 and range from 6-12. I am using the same mask, Philips Nuance Pro on both systems. If I switch back to the old AirSense 10, my numbers drop back to AHI less than 2, and no leakage.
My DME swapped out the first new Airsense 11 for a new one. The AHI events averaged 8 on both AirSense11 machines, but the leakage decreased from an average of 15 to 10. They said that some AirSense 11 units have had a leak issue. The leak is minimal, but I am very concerned about consistently getting higher AHI events on the new AirSense 11 than I was getting on the AirSense 10.
Working with my DME and my doctor, we have tried adjusting the pressure setting, as well as humidity and hose temperature, but these changes have not helped. My doctor said that he had another patient with the same issue, and ended up replacing the AirSense 11 with a new AirSense 10 to solve it.
Has anyone else experienced this and found a cause or solution to the higher AHI events on one system vs. the other?

Yep. I have the exact same problem with the exact same machines. I’ve only had the 11 for a week. Should I contact my Resmed supplier and ask to change back to the 10.

I was about to ask for an Airsense 10 replacement when I came across a YouTube video about the EPR setting and its possible impact on therapy. It’s supposed to be a comfort adjustment , but apparently can impact your therapy.
Both my 10 and 11 systems showed EPR as on so I didn’t think any more about it, but after watching the video I took a closer look. On my Airsense 10 it was set as always on at a setting of 2. On the new Airsense 11 it was set as only on during ramp at a setting of 1.
I changed that setting to always on and 2. My events immediately dropped to a range of between 2 and 4, averaging 3, and low leakage of around 3 average. I’ve been using this for a few weeks and it seems to be working. It’s not the 2 or less events I had on the Airsense 10, but is ok.
There are comments on the video and other posts that you may also have to increase your fixed pressure setting by 1 or 2, depending on the EPR setting and individual impacts. I tried moving my pressure from 9 to 10 for about a week and the events decreased slightly by about 1 but the leakage more than doubled. I see my doctor for a follow up in a few weeks and want to discuss it with him.

I agree with your conclusion that EPR is not only a comfort feature. It is a bilevel therapy with minor pressure differences.
