        Attached are charts for the past 2 nights - I am not sure if this is helpful because it is only 2 days, but would appreciate insight.

I changed the trigger to High 2 nights ago, which seemed to help somewhat, although my goal ideally is to get my events to under 2 per hour.   

Last night, I woke up at about 1:30 am, and looked at my machine, and it showed over 11 events per hour at that time.  I then switched from High to Very High sensitivity, which apparently made a big difference (I am attributing the decrease in centrals to that), but still having centrals.

Please let me know if you have any thoughts.  The bipap continues to drastically reduce my flow limitations and to be more comfortable than the CPAP, but I am worried about the centrals.

Let's move trigger to very-high and increase pressure to EPAP min 6.0, max pressure 12.0 and PS 4.

Here are my results from last night

I ended up doing trigger-very high-  min 5, max 12, PS 4.  I tried min 6,  but it was too much pressure for me. .Maybe I would get used to it? 

Now my centrals are extremely low and my obstructives are higher than my centrals.  I believe I have the flu and I am extremely congested, so that may be another variable at play.

For whatever reason, I feel with the foregoing settings as though the machine wants me to exhale before I naturally want to exhale. Is that a pressure, trigger or pressure differential factor?  Is there some way we can tweak that?  It made me wake up a few times 

I am glad for the improvement in the centrals

Thank you, will do. Should I keep the trigger on Very High?

I have a cervical collar but I haven’t worn it yet. I sleep almost exclusively on my back because it is the most comfortable for me, but may need to rethink that

Did your discomfort arise from the trigger or higher pressure? I suspect the very-high trigger is not a problem.

I’m not certain. I felt as though the machine started to exhale before my being ready to do so, which was uncomfortable. I thought maybe that had to do with the trigger setting. I don’t want to play with too many variables at once so for now I’ll just reduce the max pressure as you advised, unless you think dropping from Very High to High could help with the premature exhale situation

Usually that "feeling" is related to TiMax setting being too short. Default is 2.0 seconds. Your last chart stats show your normal inspiration time is 1.48 seconds and 95% Insp time is 1.92 seconds, so the machine is not cycling to EPAP in any way you should feel it is driving the bus. If you want to change TiMax to 2.3 or 2.4 it won't hurt anything. The best way to judge now your machine is responding to your spontaneous flow is to view the Flow Rate and Mask Pressure graphs in a zoomed view. Your cursor posiiton will put a vertical line on the graphs so you can see exactly how things are syncing.

Please see data from the past 2 nights.  Saturday night was decent, last night not so much. I am having a hard time with the cervical collar and I end up taking it off. Is there anything else I can do to mitigate the positional aspect of the OSA? Would trying a wedge pillow work?

I  increased the TMax last night, so it was more comfortable
