Experts? No experts here mate. We are simple folks who have been using CPAP for years and have been studying it through the eyes of those who came before us. It is all about CPAP users helping CPAP users.
For your specifics, it looks like the bulk of the spikes are arousals. You can see it in the flow rate and respiration rate. A wee bit of leaks, but nothing to be concerned with at this moment.
You do not need that wide pressure range as you stay between 7-9 with the current settings.
Some things to try as we figure out if the pressure support or low pressure is contributing to the arousals.
1. Set your pressure range to 7-10 with EPR 2 full-time. It is not much different from what you are using today other than lowering the upper bound. Try that and let us know how it goes.
2. Depending on how that goes, we should try increasing your pressure range to 8-11 or 9-12 with EPR set to 2 or 3. That will give us more data on how increasing pressure with EPR affects things.
Also turn off ramp unless it is absolutely necessary to fall asleep.
We may need to have you try increasing pressure even further to see how things go. Managing arousals/RERAs/UARS is a finicky beastie. It takes some experimentation to find the balance between a good quality sleep and an acceptable number of arousals.
