Keep any nasal pillows fanatically clean and make sure the headgear is a loose as possible without compromising the seal and unless you accidentally sleep on your face at an odd angle I think you should avoid any future issues.
I get that the Dreamwear shouldn't be overtightened and I won't be doing that again but thanks for letting me know that the gel pillows hold their seal well, even when loose. But I still need to find a way to anchor that headstrap. Do that and keeping the pillows loose fitting becomes easier to do so sores won't develop.
But if I did have nasal sores, the last thing I would do is continue using the pillows. What I might try is buying the nasal nose piece on Ebay or just stop wearing a nasal/pillow mask til the sore is healed. Letting it fester til surgery is required doesn't seem like a direction I'd personally care to take,,,, I hate hospitals.
