Sadly Doing it Alone!
Well greetings from Scotland!

Context. I've had horrible service from our NHS. Basically they cancelled several appointments and theres a wait list of over 7 months for a machine. 
So, I decided to go it alone as my API was 42. I bought a ResMed 10, and I'm a month in. My GP does not have the knowledge and and sadly I will not see a sleep Dr until the 2nd reschedulled appointment they have given me in September, so I am trying to navigate this myself. I will have been on CPAP for 8 months alone. (I am a retired ICU nurse so I do have some understanding of ventilation etc) However ts diffrent when its yourself!

As such I would really appreciate your help and assistance around what I've learned so far and improving my settings. I'm a month in and 
my issues are-

1. I think I have a floppy palate as my respiratory waveform indicates (see jpeg attachment)  - my respiratory rate goes up to 50 because of this I think(chart B)
 I've therefore put up my EPR is up to 3 (few CAs creeping in) I also raised my lower pressure to compensate. 

2.My flow limitations are low but constant.  What should I do about this??

3. I was getting on fine but in last week I started to have large leaks (nasal cushion) I'm going to try pillows but since I put my pressure up my nose broke down inside so I need that to heal first.

4. Anything else you see

I believe you set the EPR to ramp only.  Change it to EPR 3 (Full Time).
That should help with the flow limitations.

Don't worry over CA's right now.  They are few.  Also, be sure you move your minimum pressure to 7 so that the EPR can be fully utilized.  Scratch that... I see you moved the minimum pressure to 11,
so leave it there.  Just be sure EPR is set to 3 (full time).

I had the same problem with the NHS here in Wales, so I too bought my own AirSense 10 direct from ResMed. I also paid for phone guidance from the ResMed clinic in London. That was very worthwhile, I felt. You will probably get excellent help from the experts on these forums, but being able to talk to someone helped me up the learning curve.

I’m so disappointed. I’ll perhaps do the same thing when I’ve worked it out a bit more. Thanks as I didn’t know that was an option.
