Not able to achieve AHI below 1 consistently

Not able to achieve AHI below 1 consistently
I'm on auto cpap machine around 6 months, I tried five different machines, resmed airsense 10, airsense 11, Philips dreamstation 1, f&p sleep style and now using lowenstein smart max, and i would like to achieve AHI below 1 consistently, from OSCAR, the data shows airsense had better results.

Are there better settings so that I can achieve lower AHI result?

Thanks for your help

Below 1 AHI shouldn't be required. Medically, below 5 is treated. After getting AHI below 5, then it becomes best comfort while maintaining low AHI.

Based on the prevalent obstructive events I'd recommend changing your minimum from 7cmH20 to 8cmH20

I'm not super familiar with reading Prisma in OSCAR, are you using any time of pressure relief or comfort setting?

Also, try to get to bed earlier, your sleep quality will be much improved if you consistently target 9-10pm as opposed to anything past midnight.

Before addressing ways to improve therapy, let’s back up to what is going on. Do you have a copy of your sleep report? If so please post the summary page (or all pages if short) and redact all personal data. That will help us understand things.

Why go through so many different machines? The ResMeds would be the first we recommend, Dreamstation 1 definitely not, and Loewenstein seems okay but is still new to some of us. If you have a choice stick with one of the ResMeds. Swapping machines does not help adjusting.

I’ll skip commenting on the Loewenstein chart. For the Resmed Airsense 11 set your min to 7, max to 15, EPR to 3. Use that for a few days then report back.

Also, please read the wiki on how to format the charts for us to review. There is a link in my sig…

Don’t chase AHI numbers. Chase quality sleep.

It would be a lot more helpful if you organized the Oscar charts as recommended in the Oscar wiki.  As they are now, the most important items that we can help you with do not show.  Also, it would be good if you turned your patient mode on in Oscar.  Right now it is turned off.  Turning it on will show more useful information.  And yes, stick with the ResMed machine.  Theirs are the best.
Machine:  ResMed AirCurve 10 Vauto
Mask:  Bleep DreamPort Sleep Solution

my baseline without treatment is 50.9AHI, tried several brand to achieve lowest AHI, from machine report Resmed is the best, but from a youtube channel, some said lowenstein algorithm the best, so i'm confused. my aim is to have better treatment result. right now i have both airsense 10 and lowenstein

Based on my opinion, the ResMed may give you the best of everything regarding treatment and comfort. I myself have only used ResMed, or 2 Philips Respironics machines for a short time.

Also my opinion, ResMed represents the highest quality machine and treatment available, including to yourself.

Any AHI number you can get with consistency between 0-5 and be as comfortable as possible will likely be the goal. Let's try the ResMed followed with an overnight OSCAR chart with standard view.

Lowenstein has the best APAP algorithm as it's the most responsive but what is most important is what version of therapy makes you feel the best.

If I were you I'd probably try to titrate yourself on the Resmed because the support here is prevalent then transfer those settings to Lowenstein if you find it helps.
