Tuning a S-box
Hi all,

My first post so a bit of an intro :-) 

47y old man with OSA and UARS, worked in engineering/tech all my life, very much on the spectrum.

Have been provided a Sefam S-BOX by the NHS after a sleep study, they haven't touched the settings and I had been really struggling.

Found out how to enter clinical mode and upped my min pressure from 4-6, sooo much better as I now no longer feel air starved whilst trying to get to sleep, however I have and continue to be woken in the night by massive leaks caused by high pressure >13.

I have downloaded the Sefam software and my 90% pressure range seems to be about 10.6.

I would welcome any help in trying to tune this via the data as the NHS haven't done anything in 6m other than send me a letter saying 'well done and keep going'.

Welcome to Apnea Board,

As best as I'm able to decipher your chart, leaks are needing addressed as one of the two foremost issues.

Leaks are typically from a mask that's not sized correctly for your facial features, or from an improperly adjusted strap set. A common mistake is to adjust tight to control the leaks, and it actually makes it worse as the cushion gets crushed and distorted.

Secondly, it's possible you have some Positional Apnea patterns within Obstructive Apnea and Hypopnea graphs. This is due to the clusters of several events in a row. Examine your pillow, is it several or thick? It may be forcing your chin to tuck. Or if side sleeping, your head can be tucking towards your chest. If applicable, you need to physically prevent this by blocking the chin tuck.

Thanks Dave, I should have probably mentioned I have quite a thick beard. I have several mask options inc nasal pillow and cushion. Wasn't aware of the chin tuck being a thing and I do tend to sleep with my head tucked, at least to get to sleep.

A beard may make things a bit harder, but not impossible to get a mask seal that's decent. Use the mask link in signature. Both links go into the Board wiki.

Did you have Central Apneas (CNT Apneas ?) in your sleep study?  If you don't know, please post a redacted copy of the study.  If you don't have it, ask your doctor for a copy.  If there were not a lot of CNT Apneas in your sleep study then yours are treatment-emergent and will lessen on their own as time passes.

Happy to share my sleep study. Certainly my AHI is vastly reduced since getting the CPAP, I just want to stop being woken up 3-4 times a night.

I am actually beginning to think the leak data from the S-box is crap, I had zero discernable leaks last night and it still tried to keep bumping the pressure on me, the data shows high leaks but the only leaks were out of the vent in the mask.

With wifely permission I am self funding a AirSense10 combo whilst they are on sale in the UK so I can start using OSCAR etc.
