Quitethe1 Therapy Thread from CPAP to ASV

RE: I need sleep badly! Any help with OSCAR is appreciated
I doubt she will budge.  If I find a machine, and I have another on the hook that I am buttering up, I will just tell her that I want to just stay with the 10 autoset and deal with the headaches.  Then she won't change the supplies prescription.

The thought of having someone watch me sleep again creeps me out.  And you are right about the results. Look at my data before last nights.  It was horrible and last night was so much better.  With my luck nothing would show up that would concern her.  Then I would have a copay and study bill  that would concern me!

I buttered her up and just purchased it. It’s from Oregon, about 30 miles from where I was born. Excellent area, super nice people. Talked on the phone. 50 hrs on it, $750 + $50 for shipping. Probably too much but worth it. They are mailing it today so get ready. Canceling my sleep study.

Way to go! Good luck with the purchase. Assuming you get it, we will demonstrate efficacy and that takes care of any future titration bill.

Assuming it get it!?!?!  Did I mention they are from Oregon?  I know there are a lot of tweakers out there, but…in the outskirts of Dallas….hopefully. It’s not Portland, that city was beautiful, but man, not now.

I used to see these transactions as 100%, but saw one or two go down in flames. The odds are good, and it seems you know something about who you're dealing with. I have purchased Craigslist machines, but kind of got turned off when a member got junk instead. It was self-inflicted not using a safe payment method.

WooHoo!! I got my machine today and I am ready for 100% help!

They even sent masks that were sealed. The Dreamwear under the nose thing that I wear sometimes and the P10 that I wear never. I might know someone that might want it hmmmm..... They sent 2 hoses that were out of their package, but were the slimline. I am assuming that I can still use the hose I have now? I am using the Climateline Air heated hose. They sent a brand new water holder in the box too. Travel bag and manuals. Great people obviously!

I erased all the data on the machine...after looking at his OSCAR. He never wore it more than 2 hours at a time for 3 weeks. Shows 50 hours. Hope that won't be me. I restored the defaults as well. Ok, I will shut my yap and wait for my email that says help is here!

Sounds like you got a good one. Glad the Oregon transaction worked out! As far as help, I originally laid out suggestions for settings here https://www.apneaboard.com/forums/Thread...#pid507172

Quote:Based on your history with CPAP, you normally control obstructive events with EPAP 6.0, and have generally comfortable results with PS min 3 (same as EPR 3). The titration of maximum EPAP and Maximum PS is a simple matter of trial and error and is quickly determined by observation. The machine maintains an overall safe and efficacious therapy pressure with its automatic algorithm, and this is actually preferable to a clinical titration for many reasons. You should request that the prescription be written for Resmed Aircurve 10 Vauto (HCPCS #E0471) in ASV Auto mode, with recommended EPAP min 6.0, EPAP max 9.0, PS min 3.0 and PS max 15.0 based on CPAP history

I'll stick with this and hope to see some results soon.
Mode: ASVauto
EPAPmin: 6.0
EPAPmax: 9.0
PS min: 3.0
PS max: 15.0

I expect to reduce PS max and may make other adjustments as we see your charts.

I turned the ramp on because I didn't know what to expect and didn't want to be awake for it.  When I did wake up because of a leak. it didn't even feel like I was even wearing a mask.  Hard to explain.

I'm suspicious about the AHI and the Flow Rate. not showing everything, but different machine.  Let me know if you need to see different lines now

I do know there were a lot of mask leaks, I was wearing a Dreamwisp, will try Dreamwear under the nose or F30i full face tonight.  I was just afraid of leaks with those.

The AHI is real, there aren't any. Once you feel more confident, I suggest you don't bother with ramp or use the auto setting so it goes away when it detects sleep breathing. We actually nailed the settings. You used the full range of EPAP and PS, but I suspect we can reduce PS max. I think we should keep these settings another night before making changes. You really transitioned to good smooth therapy after 00:30 and I think that will become more characteristic for you. I'm glad the changes in pressure were not overly disruptive...so ASV is the bomb for you, huh? I told you you could self-titrate.

Your tidal volume is a bit greater and respiration rate slower. I would have expected that as you remain well ventilated and more relaxed. I'll need your opinion, but this looks like a very promising start.

I will make the ramp shorter at like 5 mins.

I felt pressure in my chest when the ramping started, but not enough to stop me from sleeping.

Sometimes with the other machine, I would lay in bed and read with my mask on. I’m thinking this is bad with ASV since it wants to get a rhythm?  If so, I will stop.
