Advice for dialing in pressures?
Hi All,

I've been using CPAP for a few years now but am at the point where I really want to dial treatment in and see if i can lower AHI. I've attached a overview in Oscar which should help show previous treatment data. As of last week I've started to raise min pressure and lower max to find a better range. Thoughts on any way I can improve settings? It seems to be raising my AHI from around 3 to 4-5 instead of lowering it. Please let me know if I need to provide more data for you. Thanks in advance!

There are a couple issues. You have a moderately high residual AHI with periodic clustered OA events. The fact they occur close together suggests we are looking a positional apnea https://www.apneaboard.com/wiki/index.ph...onal_Apnea This can often be resolved by changing pillows or avoiding sleeping positions that lead to chin tucking. Secondly, you have flow limitations (95% between 0.06 and 0.11) that are likely causing respiratory event related arousals. We can resolve that by using a higher EPR setting of 3. Read the positional apnea wiki and share any thoughts on how this may apply. Try increasing minimum pressure to 8.0 with EPR full-time at 3 and keep maximum pressure at 14 for now. This will result in a pressure range of 8/5 to 14/11 compared to current range of 6/5 to 14/13. I think you will find this more comfortable, and if you can resolve the positional issue, we should see an improvement in AHI and sleep quality.

You folks are a blessing to us who are struggling with CPAP! I'm working on getting my sleep under control but I'm not giving up yet! My graph looks like a horses mane with a bad haircut!  Laugh-a-lot Don't give up sleephead777!

Leaks are out of control here, and we will see much more valuable information if the monthly calendar is minimized. Just click the triangle in the date line. Happy new year.

Hey Sleeprider, Thank you so much!!!! I made the suggested changes and have seen a pretty massive improvement in AHI, screenshots below. I also got a new pillow the same day as making the changes so I think a bit of both have helped. How are things looking now, any room to fine tune additional settings? Thanks in advance!

Well 777, well done! That's a lot of progress. Still some positional obstruction, but much much better. I'd like to see you try +1 cm of minimum pressure to see if we can clean it up a bit more.

Thank you so much, sir! I used the new Phillips Dreamwear nose pillow two nights ago and numbers were even better. Unfortunately, I woke up with serious under nose pain. It reminded me of 2nd grade when my teacher, Mrs. Trout stuck her fingers in my nose and dragged me to her back office for eating paste! I also got what looks and feels like a painful pimple between my nostrils. I switched to the full face mask last night and I have shared screenshots of the results below.  Big Grin
