Could I benefit from going to BiPap VAuto?

I'm just catching up. The reduction in pressure had better results and seems to be more comfortable. Pressure is more stable and respiratory statistics are about the same. I think the minimum pressure of 8.0 is working well. What do you think?

Thinking about it and I want to make sure I'm expressing myself right,  when I start the machine and before I'm sleeping, I have a small feeling of "missing" air on inspiration right after the expiration for a split second on some inspiration.

What could be causing this?  Is it the lowered EPAP with the PS at 4?

I guess at this point we're just fine tuning.  I wouldn't want to be searching for things that are not there.  Is this when TiMax and TiMax comes into play?

You are already on very-high trigger sensitivity, and this has nothing to do with TiMin or TiMax. There is not much we could do beyond increasing pressure so EPAP is higher. With full-manual controls in S-mode we can set rise time, but then you don't have Easybreathe and get a more square-wave pressure transition. I don't have a solution. In your zoomed image, there appears to be a brief loss of pressure as inspiration begins. Try experimenting with high trigger rather than very-high and see if that feeling is relieved. Other than that I just don't know.

Hi Sleeprider,

I wanted to take a few minutes to thank you for all your help.

I reverted back to PS:4 with 8-18 pressure at very high trigger.

I feel great during the day and I don't think we can improve much further.

   


This is a great community and I should have taken the plunge wayyyyy sooner to ask for advice.

Is there anything that you see for my last night of sleep?

   

The reason I'm asking, is that I had to take half the day off today as I had so much brain fog that I couldn't think straight.

I see some "peak" in the Tidal Volume, but that does mean anything?

You could try titrating the EPAP min up to avoid the OA evens. I don't know how much of a problem that is, but they all occur at the minimum pressure. Maybe try EPAP min 8.6 or 9.0 and see if that makes any difference. No other changes needed for now. Trigger is already very-high, so unless you feel out of sync, that's fine, or you can try going back to high.
