UARS With BiPAP - Looking for Some Guidance
I was diagnosed with UARS/mild sleep apnea last year (pRDI of 9.4 and O2 nadir of 91%). I was originally prescribed APAP therapy, and had a very low AHI but no improvement in tiredness.
To improve tiredness, I am now on an AirCurve 10 in auto mode with a PS of 4.2. Unfortunately, I am still tired after a few weeks using these settings and was hoping you might have some advice for improving my tiredness issues.
I have included a couple of screenshots from OSCAR below. The first is from a full night, and the second is zoomed in on the flow rate during a period of (what appears to be) relatively stable sleep.
Thanks for any help you can provide!
RE: UARS With BiPAP - Looking for Some Guidance
I would like you to try adding a bit more pressure and reducing pressure support a tad. Something like:
EPAP min 6 (you could use 5 if the 6 feels too high)
IPAP max 12
PS 4
Trigger very high
The trigger sensitivity should handle the CA’s and the pressure should help smooth out the flow rate without hopefully causing more events. I think pressure changes responding to things should be minimal.
We may need to tweak up or down a bit depending on how things go. Give it a try and let us know how things go.

Thank you for your guidance! I will give these settings a go starting tonight and follow up.

Hello! Yes, changing the trigger setting really helped me with the clear airways. I have also increased the pressure support over time. However, I continue to experience tiredness issues. I am not sure how much of that, if any, can be attributed to the machine settings though.
My current configuration is:
Min EPAP - 4.2
Max IPAP - 25.0
PS - 6.4
Ti Max - 4.0s
Trigger - Very High

Yes, I would say that I do. I am also sleeping more soundly throughout the night and not waking up as much.
