Thanks very much. I'd appreciate any advice on mask setups etc. I thought the N20 would be a good fit for me, but that doesn't appear to be the case. I live in a rural area, and don't really have access to any sleep therapists these days, so I'm grateful for any advice you folks can provide.

Do you happen to have any idea why the sleep report on my machine says the pressure is higher than 12? It varies between 15 and 16, and I guess I'm not understanding what that number actually represents.

Some comments…

1. Do you have a sleep study? If so please post the summary with your personal data redacted.

2. You have quite a few large leaks as G.Szabo pointed out. There are a number of good videos on YouTube around mask sizing and setup. Try starting with LankyLefty27 and work down the rabbit hole if needed. It may also be that it is the wrong size or just the wrong mask for you. Many of us go through a number of masks before finding one that works well for us.

3. Do you feel like your mouth is dry in the morning or does it wake you up at night? If so your leaks are from your mouth, so consider trying a soft cervical collar or mouth tape. A flatter pillow may help as well. If that does not do it then you need to try a full-face mask like a ResMed F20 or F30, or an F&P Evora. With the FFM if your mouth drops it does not matter as it will still be inside the mask, unless it full opens.

4. You have a lot of flow limitations which are driving up your pressure. Try the following settings:

Min pressure 10
Max pressure 16
EPR 3 full-time
No ramp

Note that you may have more CA’s and possibly other events as a result. Depending on how things go we can adjust further from there.

5. I cannot comment on your question about the pressure going above the settings without seeing an OSCAR chart for that night. The chart you posted shows it responding correctly to the settings.

Thanks very much for the detailed response.

1. No, I was diagnosed at least 30 years ago, and have not had a sleep study since that time.

2, I will check out the YouTube channel you suggested. I have run mask tests, which don't show any issues. The only leaks I've ever noticed with my current mask is at the top of by nose, just under my eyes when I turn my head sometimes. I do sleep on my back, and did tighten up the head gear to get rid of that particular leak (as far as I'm aware of).

3. No, my mouth is not dry in the morning, nor does it wake me up at night. Nothing about the process wakes me up. I am using a medium ResMed N20 (a new one I just bought to replace the same mask which was 6 mo. old).

4. I made the setting changes you recommended and am attaching a chart with data based on those changes.

Thanks again for your help, I've kind of been on my own with this for so long it's really nice to get some knowledgeable assistance.

I've made some adjustments to the mask and brought the leak rate down. I'm using a Resmed N20 Airtouch and I'm getting a better seal beneath my eyes.

I'm attaching some OSCAR charts in hopes you might have any further suggestions/recommendations for me.
