APAP optimization assistance

RE: APAP optimization assistance
I decided to stick it out with apap therapy for a full two months and had AHIs as low as 8 up to 45, mostly central events.  I felt like a zombie on apap most days.

I managed to find a used ASV 10 and jumped on it so I can compare the therapy.  Last night was my first night with it on ASVAuto mode.  I woke up quite a bit due to the pressure changes, but was able to sleep some and was shocked to see zero events this morning!  Attached my Oscar data for last night and the night before for comparison.  Anything I should change?

My last night on the airsense 11.

RE: APAP optimization assistance
One of the most satisfying things I do on this forum is to get individuals that are really suffering with the wrong device, get the therapy they always needed and deserved. Congrats! Don't be shy about blowing back and regaining control of the ASV if it tries to run the show. This is usually a temporary issue as you adapt to ASV and most people get much better sleep after a week. So far your results look very good, and I think you should stick with that another night. Once that is done, try PS max at 12.0 and decide if that is more comfortable. Your EPAP is very steady with a minimum of 5.0, and the PS 3 seems to be adequate to keep you comfortable.

Thanks sleeprider, can’t thank you enough.  Will update after another night on these settings.

RE: APAP optimization assistance
Decided to run the original settings for three nights.  So far, haven't had an AHI above one.  Aside from the random UA and H events and waking up every few hours, its going well. Do you see any room for improvement?

RE: APAP optimization assistance
Give the lower PS max of 12 a try. Before masking up, take time to let your respiration settle to a lower volume similar to sleeping. You can see that when you first mask up your Vt is up around 950 mL. The device is using those first 90 seconds of therapy to set its targets for respiration rate, and minute vent and as a result you get very persistently high PS.

RE: APAP optimization assistance
I tried the PS max of 12 for a few nights and have been getting increased hypopneas for some reason.  Last night there was a single cluster, not sure what happened there.  Other than that, I seem to be waking up less and getting used to the machine, but I'm concerned about the hypopneas creeping up.
