Been on CPAP therapy for 3 years feel worse than ever
Hi board,
As the subject states, been on CPAP for 3 years and feel worse than ever. As tired as I would be if I wasn't using the machine. I spend my days yawning.
I have tried increasing the minimum air pressure, starting pressures, I have tried changing masks, nothing seems to help.
The only thing that I can put my finger on is that when I'm on my back and drifting off to sleep, upon my exhale, sometimes, it feels BLOCKED. And when i'm really relaxed and on my back and I exhale, it actually creates the snoring sound.
Sometimes I can't even exhale. Could this be "Palatal Prolapse"?
Using an Airsense 11. Minimum air pressure 8, Max 20.
Your assistance would be greatly appreciated.

It would be helpful to see a whole night including the settings on the left hand side. Did you get the same "blocked" feeling when you were using EPR? Would love to see a zoomed in view of around 1hr30m into a night to see what your breathing looks like during REM.

Hi, does this screenshot help? Also, I haven't touched my EPR settings. I think they are set currently to auto?

There are 2 parts of the problem. First is the centrals. IF you had them during your sleep study there is not a lot we can do with the machine you have. IF you did not have them as part of your sleep study then they are treatment emergent which means that they should mostly go away as you get use to the therapy.
Your EPR is doing nothing because you have it set for ramp. Please change that setting to FULL TIME and turn off the ramp if at all possible.
That is not much of a change but I would like to see how your centrals do with the EPR set full time at 1 before trying other changes.
Post again tomorrow with the changes and we will see what else needs to be adjusted.

Hi, thank you so much for taking the time to review and reply. Please excuse my ignorance, where are you seeing centrals? I was never told I had central sleep apnea.
I will set EPR to FULL TIME and turn off ramp. Will post again results tomorrow. Thank you again

Yes, but there was no mention of me having central sleep apnea on the reports. Maybe I developed after? This is scary stuff...

Central Apnea isn't as troublesome as the Obstructive events. That is unless you have a lot. The Centrals can be caused by lower CO2 via the CPAP flushing it out at higher rates than the brain is expecting. Another reason can be a nervous system medical condition. Finally, like what I have, would be idiopathic, unknown medical cause.

CA are central or clear airway. you have several. central can NOT be controlled by pressures. If you had centrals in you sleep study about all we can to is try to cut them back but we will not probably be able to eliminate completely. All a central is you holding your breath for 10 seconds or longer. We all do that while turning over in bed or when you pick up a blanket. They are not a huge problem untiol they get numerous.
