BiPAP new therapy
#51
RE: BiPAP new therapy
Hello everyone.

I am very happy with the progress I am making.

For the past few weeks, I have been working on the PS.
I am including a screenshot that shows how AHI and Flow Limitation have reduced.
The dates are from left to right contrary to how Oscar sort them.

I am also including my last night results which are the best yet.

I think I would try increasing more PS and see what it does.
If it is not good, I can come back to last night's settings.

Any thoughts, comments or suggestions?

Wow Dan! My 0.00 days are few and far between. Do as you feel works best for your therapy, but don't fall into the trap of chasing the numbers. How you feel is the way to measure therapy now.

Yep, my thoughts also.
I forgot to mention that I do feel good when waking.

Because I am curious and think the measurements are an important part, I will probably test some more.
But I am conscious not to forget how I feel.

If I find something interesting, I will share it here.

I am very, very grateful for all the support.

Great work you are doing!!!

RE: BiPAP new therapy
REM is not an apnea topic. I may just have to control my dream outbursts.  Pardon me.  NIH has much to say about breathing differences in REM vs. NREM.  Tidal volume is generally noticeably lower than NREM sleep, the body seems to be slack at regulating the respiration rate and tidal volume allowing wider variations.  The tendency is for breathing to get more shallow and respiration rate to be higher, and both widely fluctuant.  In addition, the wave form of respiration is sinusoidal and generally without gap between the exhale and the next inhale, so it looks much more like awake breathing than NREM sleep breathing.

https://www.ncbi.nlm.nih.gov/pmc/article...41/?page=9 shows a graph of Vt lowering from NREM levels during REM.
9Ironically, this paper is about brain blood flow in various sleep stages.)
See also Para 2.3 of https://www.ncbi.nlm.nih.gov/pmc/articles/PMC3199805/

I will post a couple of screenshots of what I believe is typical of my REM sleep.

In the meantime, check out your air flow between 05:00 and 05:15 on Sunday night Feb 12.  also, between 06:50 and 07:00.

RE: BiPAP new therapy
I'm setting up my Excel to track my results on the long term.
(I like to see graphs showing all at once).

Looking in the exported CSV file, there is a column for 95% EPAP.
I don't see a column for 95% IPAP.

I am assuming 95% Pressure is the IPAP.  Am I right?

Also, still in the CSV file, all columns with the word "Set" in their headings show "0" in all lines.
Since EPAP and IPAP can be set to different values in time, it would be nice to have a trace in the CSV file.
(I know I can get the values from Oscar, but I am lazy...)

QAL, that is a bit too much for me.
I am more of the KISS kind of guy.

My recent results are good enough to stop looking for more ways to improve.
If things go to hell, I will be back here with questions.

ah. hate to go to that much trouble to explain it, and sorry it seems like too much. just thought you'd asked, and trying to do the best at explaining. maybe it will come in handy some day to someone.

I am reposting this one because I am not certain it was picked up.

RE: BiPAP new therapy
Since I don't use the CSV option, I'm basing this on nothing but logic. For the 95% IPAP, just use the EPAP value and add your PS setting to it.

RE: BiPAP new therapy
I am in test mode with the AirCurve and found something that intrigues me.

Last night, I had the PS up to 7 to see what would happen.

This morning I feel just fine.

Now why do I have FL in the second session?
Nothing changed between the 2 sessions.
95% Flow Limitation is 0 (zero) for the first session and it is 0.21 for the second session.
0.21 is 36% less than my overall FL when using a CPAP. That is good news.

As I understand FL, the main culprit would be the airway constriction.
There's not much a CPAP or BiPAP can do to eliminate that constriction.
Constriction must be the result of the position of the head, thus the need for a cervical collar.

I will receive a new collar in a few days.
I bought a different type with the hope of better head motion control.
Then I will test everything again.

Am I beside the track here or am I making a reasonable move?

Thanks again for for your patience with me.
