Let me give a quick reply on the mask issue.
You are doing quite well with leaks at whatever mask you were using in your most recent chart. Good job! If you were using the F40, adjust the setting on the unit to pillows. I have no idea why, but that’s the setting Resmed specifies on the packaging of the F40. Also, it might help to consider the position you are sleeping in. See the string of OSA’s in your chart? Could your neck be kinking? A kinked neck could happen if you are using a high pillow, sleep with your head towards your chest while on your side, etc. if you visualize, a kinked neck will cause an obstruction. For some folks, this can also happen if they sleep on their backs, because the jaw slacks and the tongue flops back. This causes OSA’s. The unit might attempt to address this by escalating the pressure, but there’s no pressure that can respond to positional apneas. The only “treatment” is to find a different position, where your neck isn’t kinked and/or your jaw isn’t dropped. This takes trial and error. But fixing it can keep the unit from zooming upward to try and address an OSA that it can’t actually resolve, which in turn can mess with your sleep with arousals.

Hello Starwalker, you are having many CA and OA events in that last chart, your min EPAP pressure at 5cm seems too low to me, I would raise it to 7cm and see how it goes for a few nights.

Here is last night's chart. Doesn't look great (to me) even though AHI is low but I'll leave it to more knowledgeable experienced minds.
Thanks for everyone's input yesterday, I made the setting changes as suggested. Half-Asleep-you are right on that I use high pillows. I actually use 3! because my neck is very arthritic and painful unless supported in a certain position. Unfortunately, I had the same problem with the F40 causing facial dents, swelling and my face generally looks all messed up. Very upsetting. Reason is that I still have to pull it tight to stop leaks aa all the other masks. I have very sensitive skin and it just dents/marks easily. I had great hopes this mask would be different, but no. Yes it's softer and all silicone so hard plastic isn't sticking in my face but pulling it tight produces the same results from the straps and frame. Maybe it's just my face and I can't wear face masks but I can't do the nasal thing as I am a mouth breather. I don't like walking around with my face like this so will probably stop the therapy again altogether. I have now tried just about every mask. A shame because I actually am breathing better in the morning after using it for 3 days. If anyone has any idea how to solve this problem I would be most grateful. Thank you!

You are in a tough situation with your arthritic neck. Using a wedge pillow (standard size pillow) would probably cause your chin to tuck into your chest even more and make your obstructive apnea worse. You can try one though if you like. The incline angle of a wedge pillow may be less the incline of you current 3 stacked pillows set up.
Currently I sleep on a full body wedge (36" x 36"), but I keep my head tilted back (I use a very very very thin pillow). Even on a full body wedge, you would probably have to use your 3 pillows with your arthritic neck. Have you tried mask liners on your mask. They help some people with comfort (and even a better seal also). Your choice if you want to try these also, of course.

Thanks Half Asleep (great name!) Yeah tough situation for sure. You’re right about the wedge- I got one from Amazon and still had to use my 3 pillows and it was just completely uncomfortable.Gave it to my husband to elevate his legs at night, worked great for him so didn’t go to waste. Haven’t tried a mask liner-might help with redness but not sure if it will stop it from denting my face when I pull it tight. Worth a try though. Thanks for the suggestion. I think maybe because I have really high cheekbones the masks just can’t confirm so get air blowing out the sided sides.
Btw, what did you think about most recent chart and would you recommend any setting changes? Didn’t use it last night so the redness and swelling subsided some. I’m still breathing better after using it for a few nights so know it helps me hoping I can find a mask solution, then maybe can try positioning my head more to the side? Usually don’t move all night. Argh, so frustrating! |
