Definite marked improvement. Became a bit too much, but it was more me overheating with something on my face than anything else. 

   

Thinking about increasing the EPAP again. Also was taking a look at the Alaxostent, seen that there's been some luck with its usage but price is a bit of a pill on a risky bet.

If you can grab a zoomed shot of about 4 minutes in duration, it might help to see what is going on as the hypopnea event clusters initiate. I would be looking for evidence of airway obstruction from obstructive chin-tucking.

If you read the positional apnea and soft cervical collar wikis, you will see other examples of members with ASV having positional obstruction. I think you can view the mask pressure chart along with flow and see the machine is delivering high pressure support that is not getting through. This is very flow limited and appears obstructive. Cervical collar(s) are coming Thursday -so I’ll give that a try. In the meantime I did give low/no pillow a try with pretty similar results to what I already had.

Are there any signs on that limited data of expiratory prolapse? I compared it to the chart of someone with an actual or perhaps more severe prolapse and didn’t necessarily see the signs. Regardless, every time I’ve been getting a full-on open eyes arousal it’s been my palate forcefully flapping forward and ‘clapping’.

The most likely image of prolapse is this one. After that second hypopnea we see expiration terminating with an abrupt positive pressure pulse, then zero flow ahead of the next inspiration. That pattern is consistent with PP with flow shunting from nasal to oral expiration.
