CSA?

Question
CSA?
Dear community,

I'm using the PAP therapy for somewhat 4 months now.
While my AHI was 2.X 3 years ago it has been 10.6 this year.
So I got the AirSense 10, everything was great, just the usual small problem's not worth to mention.

But since last week I'm always getting waked up by my body with an adrenaline reaction.
First I thought it would be the stress I have at the moment, but while doing some breathing training to relax before sleeping I noticed that my mask was "stuttering".
Normally this happens if you don't breath for a while, I think to measure if it's an obstructive or central event.
From this moment on I always have the feeling that I was not breathing when I got waked up by this inner shock again.
Believe, if you are absolutely tired, this is driving you insane and causing heavy depression and fear of sleeping, when this happens again and again while falling asleep.

So I have checked my daily reports in OSCAR and found some weird airflow while those moments occur.

Now I'm afraid that this is a CSA, especially because I have vertigo since 2 1/2 months.

There are just some few CA events in the night, minus the ones when you rotate in bed while sleeping.
But when trying to fall asleep they are happening multiple times in a row?!

Is it my psyche or is this a light CSA or something else?

I also had an polygraphy this week, I remembered the exact time when and event occurred and there was an amplitude on the result paper, but the doctor said this would be an "artifact".

Maybe someone has experience with those datas in the pics or the situation itself.
The pics show different arousal events while dropping off the last nights.

Any help is very much appreciated Smile
Thank ya'll in advance!

@nonplusultra,

Welcome to the board!

Glad you've got OSCAR for for analysis, to get the best help available you'll need to arrange your charts in a specific way, the link with help doing so is below -

https://www.apneaboard.com/wiki/index.ph...ganization

Get those charts organized and repost some screen shots to this thread folks will be more than happy to help out. Full zoomed out to begin with is a big help, people will let you know what to zoom in on and how far.

Welcome to the forum

In those views I see nothing I'd classify as central, just a lot of Flow Limitation and Snoring, no arousals.
Please post a full night view as that does tell us a lot.

RE: CSA?
Thanks for your replies.
Hope those screenshots suit your needs.
(I've changed from APAP 6-8 with NM to CPAP 8 with FFM - using this for 2 nights now, but nothing has changed yet with the arousals)
(Sorry about the language, haven't found a drop down to change that in the settings)

RE: CSA?
Those are the diagrams from last night.
I've incremented the mbar by one and activated EPR 3.
Seems those CA events occurred more.
For this night I'll install a cam to determine the CA events which are caused by movement.

@nonplusultra,

Looking at these charts I can't tell if you need to change the language on the machine or in OSCAR. If you haven't yet, you can change the language setting in OSCAR by going to the Help menu, its the fourth selection from the right in the very top set of menus in the upper left of Oscar right below the OSCAR version id number. Select help then choose the fourth option from the top, this will open the Language change dialog. Only giving detailed directions because I don't know whether or not OSCAR is currently in a language you can read because of reasons unknown. If OSCAR is in English and you need to change the machine to English so it will display as such in OSCAR then that is in the Clinical menu of the device. Use your preferred search engine to find "access clinical menu or airsense 10" that should get you there.

I'm struggling with analysis due to language but the results of attachment 51739 shows to be best of the lot for Flow Limit management, several of the CA appear to be sleep disturbance related rather than true. Hard for me to tell, others far more skilled than I are looking in. If you get the language changed it will probably get more comments.

Good luck! Keep at it, that's how we get better at it.

@nonplusultra,

Looks good! One more step for board chart config, click the little down arrow (carrot) in the left of the date box at the top just under Welcome, this hides the calendar and moves the data up, for those with low res screen it reveals more data, in your case most of it is there. No need to repost this time, got enough to go on, just for future attachments.

This attachment looks great settings wise, Flow Limits are a smidge high. Some of your previous attached screens had some pretty significant Flow Limits and leaks that need attention, and it appears you did.

Most of those CA events appear to be positional or from sleep disturbances, like rolling over or adjusting a pillow from this zoom level. Except the one second from the right, it could stand a look zoomed in to about a five minute window. 7th and 8th from the right might also deserve a closer look. Other than that its like Gideon said.

We strive for Flow Limits below .06, .03 is getting dialed in there, you are headed the right direction.
