I never replaced it.  If the silicone looked out of shape or not quite the oval/round shape, then I would.

I take it out, it was in the ResMed cleaning instructions (once a week, along with the hose, water reservoir). It has to go back in correctly, though, or the humidifier doesn't seat correctly!

I take the Tube Outlet Connector out and clean it every time I clean the machine which is once per week.  Once per week is probably overkill but is what is recommended.  Have never seen any damage on the piece.

It's very easy to remove and inspect. I have even run it through the dishwasher, which seems to restore the shape and seal.
