Evaluate me! Also - my provider and his hogwash...

Evaluate me! Also - my provider and his hogwash...
First of all, I'm posting my OSCAR study.  I've been at this for 4 nights.  I wrote my provider, and he first told me not to change the settings on the Dreamstation Auto, that only a licensed pro can do that (bologna, I know.)

But he also told me "From your therapy on the 6th I see your mask leak is at 18.4, which is a very good fit. I would stick with that size. Your AHI has dropped from 93.7 down to 13.8. Which is exceptional and very good therapy. With severe apnea, you should not expect you AHI to drop into the normal range of 5 or less."

Are you telling me i'll never have a good AHI range then?

Attached are my results from last night.

My sleep study test revealed that I had an AHI of 70 during normal sleep and an AHI of 60 during REM.

My very 1st night of using my CPAP at home and my AHI was under 5. I think it was 4.3 to be exact. From the 2nd night on, it was quite a bit less. My lowest AHI so far after approx. 8 months of treatment, has been 0.3 I've hit that mark several times.

It says you are on min 5 max 20, but the machine doesn't rise above 7. I'd raise the pressure to min 7 and see if you can kick the machine into gear, they are slow to respond. I think you need more pressure. Standard is that you raise min pressure till the OA resolve. There may also be some positional apnea for some people.

There is advice at the top of this page, on how to display the OSCAR daily chart
mask fit http://www.apneaboard.com/wiki/index.php...ask_Primer
For auto-cpap, from machine data or software. You can set the min pressure 1 or 2cm below 95%. Or clinicians commonly use the maximum or 95% pressure for fixed pressure CPAP, this can also be used for min pressure.

Can you validate your settings on the machine. It should have gone over 7.

Keep everything in one thread because that will make a history for you.

Your obstructive events are clustered indicating a positional apnea typically from a chin tuck. Many get this resolved with either a loose fitting soft cervical collar (releaf is popular) or an anti snoring collar, no back of neck support, ( Dr. Dakota is popular). Looking at the chart you obviously need a higher minimum. Set your min pressure to 8.

Your Duck, most notable by the sound they make, said you couldn't get to 5 AHI, well I started in the 90's and now I'm a typical .5.

You need a new Sleep Doctor.

My bad - i'll keep everything in the same thread from now on!

Can you tell me what you mean when you ask "Can you validate your settings on the machine. It should have gone over 7."

I have changed my minimum pressure up to 7 for tonight.  I also have a new mask for tonight - the Dreamwear full face, got a good deal on it.

We'll see how it goes tonight.  I know it's a marathon to figure this out for some.  I'm very overweight (6'2", 300 pounds) - with a fat roll on my neck.  That may cause some of the problems.  But i'm going to get this down.

By reading the settings on the machine and not using software,

Can you make sure opti-start is not enabled on your machine, if it is please turn it off to get full benefit of treatment as something seems to be limiting your max pressure, you may need to go into the clinical menu to see this information

INCOMPETENT, NEGLIGENT, OR CRIMINAL...HE HAS TO PICK AT LEAST ONE OF THOSE.

Are you telling me i'll never have a good AHI range then? NO, BUT HE IS.

Attached are my results from last night.

I think this person should be reported to the college of physicians or to the state board.

It's so much easier for him to leave his patients at <15 because, you know, the machine does that automatically for him.

So stick around with us for a bit and after a few adjustments you'll be able to go back to him and stick a <1 AHI report up his bum.

Well last night sucked. I used my new dreamwear full face mask.

I thought it was doing well. I felt like I was breathing well through it before I fell asleep. However when I woke up this morning my AHI according to DreamMapper was 22.5!!

I had put my minimum pressure at 7. My 90% pressure was 8. Seems like I’m going to have to figure this dang machine out more and do the “illegal” stuff to it.
