I'm back, tired, and hopeless. But is Bilevel the answer ??

She said RERA/UARS does not really exist...

And she asked me to try to sleep without CPAP or dental appliance for a moment, to check how I feel. Because my initial exam showed AHI 24 which is low.

Disappointing.

Your doctor should take up arguing the World is flat.

If I ignore the medical advice of my "sleep doctor" what do you think : I keep my Resmed Airsense 10 with EPR 3, or I try Resmed Aircurve 10 Vauto ? I have the oppoturnity to get one at good price...

I used to use a CPAP for a little while and hated it. I would feel better, but I'd fight with my CPAP all night. Often I would take it off at night thus rendering its effectiveness somewhat useless.

I switched to a BiLevel machine, the Philips Dreamstation, using the Auto BiLevel with BiFlex (not just BiLevel alone but also *auto*). My sleep has dramatically improved. Also started using a nose mask compared to a full face mask. Nose mask is so much more comfortable and easier to manage.

I really like it because it makes exhaling easier. Feels more natural. Plus of course I feel much much better.

Hope that helps!

Thanks, I really want to take a chance with Bilevel.

Now I will buy it without medical advice, that is uncommon in France, and I don't know which settings to apply (I think something similar to Airsense 10, like EPR 4 or 5  Smile )

I'm not an expert, but I would think similar levels.

For me, I just adjusted the levels over a period of a few nights until I got it right. At least with the Dreamstation, there's an app that will connect to it and show you some data, the min/max pressure support adjustments will affect the min/max IPAP/EPAP, so it can take some tweaking.

When I got it right, my AHI improved 80%.

In fact I ordered the Bilevel today because anyway I did want to get a backup device when I am traveling.
And I could have a good price.

That's why to I wonder how to set the Aircurve 10 like Airsense 9-13 EPR 4 for instance ?

S9s only go to 3 on the EPR. Based on this, I will provide pressure settings based on 3. Set your minimum EPAP to 6, your maximum to 13 and the PS to 3.

This kind of thing makes me realize how lucky I got...eventually. My sleep doc ordered what I understand to be a pretty thorough polysomnogram, and based on the results diagnosed me with UARS based on the combination of elevated RERA despite low AHI (and of course, excessive daytime sleepiness). He's a specialist with a lot of experience, with Kaiser - not exactly a temple of experimental or novel treatment. Of course that was only after about 10 years of my general practitioner brushing my symptoms off as "depression." I can't tell you how many times I told doctors "If I'm depressed it's because I'm sleepy all the time, not the other way around." I only was able to get treatment after switching doctors several times until I got one who was able to listen when I said "I'm pretty sure people don't fall asleep at red lights simply because they're depressed." It is sad that so many people suffering from "idiopathic illnesses" are just lumped into the mental illness bucket...as if living with some of these symptoms for years wouldn't drive anyone crazy! Hope you are able to get some relief weuw, I can sympathize.

I wanted to compare my Airsense 10 which is set around 9-13 and EPR 3.

So on the Aircurve 10 I will use EPAP 9 IPAP 13 with PS 3 or 4 for example ?

RE: I'm back, tired, and hopeless. But is Bilevel the answer ??
No. EPR is subtracted from the IPAP pressure, whereas PS is added to the EPAP. Set EPAP = 6, Max = 13, and PS = 3. Using this setting will give you a IPAP range of 9 to 13 (6+3) to the maximum pressure setting (13).

Using a starting minimum of 9 plus the PS of 3 = an IPAP of 12, and if you use a PS of 4 would yield an IPAP of 13. Using your setting of 9-13 PS 4 would give you a static pressure setup.
