The extreme hypoventilation was definitely due to alcohol, but I am having events of central alveolar hypoventilation even sober. I guess that is why my sleep study only highlights hypopneas (there is not a single OA nor CA).
It was quite obvious with the Dreamstation CPAP, slightly less obvious now in VPAP since the IPAP helps to maintain a constant flow rate.
Just got my pulmonologist, who (like you Matt) said there is no consequences at all if my tidal volume goes up to 440ml per night.

Sounds good. I think you did a good job, in that we usually just want to raise our pressure enough to achieve our goals, but not go any higher than that.
What size nasal pillows are you using? When I used the P10 Pillows, I found that the largest size felt the best (like I was getting enough air) and leaked the least.
You can get tested by a neurologist or pulmonologist if you start feeling symptomatic during the day. Simple tests that are cheap and doesn't cause any pain are Transcutaneous CO2 monitoring, spirometer and peak flow meter. The best way to test if your diaphragm is dysfunctional is they can measure your Forced Vital Capacity (FVC) while sitting and lying down. If you truly have an NMD and also normal lungs, a full Pulmonary Function Test (PFT) is just a waste of money as those tests can't account for diaphragm dysfunction and should really only be used for lung/airway issues.

It's been less than a month since I posted my concern for your hypoventilation and suggested VPAP ST as a possible solution. http://www.apneaboard.com/forums/Thread-...#pid272127 In this short time you have made amazing progress, taking initiative to get better diagnostics and effective therapy. Your panic and anxiety attacks are gone,and by any measure, your problems are solved and your life changed for the better. This is one of the fastest turnarounds I have seen. Congrats on your accomplishment and I'm impressed with your efforts to find a solution. Have any of your doctors that originally put you on CPAP seen this? It's just amazing and educational.

Just a quick question though, I know a normal I:E ratio should be around 50%. However, if it is not the case, is it better to have it around 40% or 60% ?

All of those are within the normal range, in fact as long as expiration is equal or longer than inspiration I'm not going to be concerned. Among other things, the mach ines are highly error prone for calculating I:E, especially Philips.
