Need Help Optimizing APAP (Potential UARS?)
I just went through my 2nd night of APAP. I didn't use an SD card the first night, but also my settings weren't great night 1 because i had my settings from 4 min pressure to 7 max pressure and I woke up a couple times to see my pressure at the max threshold (7).


These are screenshots from night 2 and it seems like the pressures end up ramping up to about 9-12, but I still experience fragmented sleep and right before waking up at the end, noticed pressure was noticeably high at 13 where my mouth was very dry and pressure was too strong. I just learned about flow limit today and my flow limit is pretty terrible, any ways to optimize? I realized (pressure relief) EPR was off on my machine so I just turned it on and set it to 3.

To also note, the sleep actually started after 23:30, before that I just wore the mask in bed to get used to the machine and mask before actually laying down and sleeping.

My main severe symptoms include:

-Body constantly in fight of flight mode after a bad sleep
-Body heating up at night in bed
-Maintenance Insomnia
-Trouble falling asleep
-Greasy, oily head with hair falling out on bad nights of sleep
-Brain fog in the morning and memory is getting worse over last 6 months

I've also attached screenshots of my sleep study report (AHI of 4.6, RDI of 5 with almost 30 spontaneous arousals an hour)

Appreciate all the help I can get. Last two years have been terrible and I'm a pretty healthy and active guy. Done bloodwork many times even recently the last couple of weeks with everything coming back in normal range. Really think I have something related to my airways collapsing so going to see an ENT when I can, possibilities of UARS with this data? I'm also a mouth breather at night so I have 2 full face masks to try.

Based on all of this data, how would you guys recommend I optimize for the nights moving forward with settings and such?

With UARS you need to maximize the difference between inhale and exhale pressure. Turn on EPR full-time at setting 3. You will see a big difference. Your flow limitation is an incredible 0.56. We are looking to drop that below 0.10 and stabilize pressure.

Awesome, I have that turned on to an EPR of 3 for tonight. What pressures would you set the machine at based on the data here? Do you think min of 8 and max of 14 would be good with the EPR at 3?

I think those pressures will work just fine. I'm certain you will see and feel a big difference using EPR. Let's see it tomorrow.
