Please help with treatment review
RE: Please help with treatment review
Thanks sheepless,

Just got home from work.

Here it is

Lowered P(max) to 17
Increased P(min) to 15
EPR=2

Leak problems Bought new tape to use tonight.  I shall tape my lips.

Dave

ps Worse leakage I've seen in some time.  Used my home-made pad under my chin.

Neverhad such bad leakage
Feeling---so tired.

Woke up at abut 5:30....tightened the top straps on my mask....

Last night I taped my lips shut.  Right across.  New tape seems ok.  I used my pad under my chin.

Sleep report on iWatch shows I woke fully at 11:00 and about 2:30

I'm so tired.

RE: Please help with treatment review
well, we can see that it isn't apnea that's leaving you feeling tired. great ahi! the zoomed views of shallow or flow limited breathing are suspect because they're in the middle of some big leaks. I've never understood the logic in the assumption that mask leaks are spikey and mouth leaks are flat topped, meaning mask leaks are very brief and mouth leaks last longer, but if that's true, taping isn't working for you.

I know you've tried a lot of methods to control leaks, some successful for a time, but not consistently so. are you using a collar? the taller the better. they're a pain in the neck, literally and figuratively, but it's the only way I can keep my lip leaks down.

I wonder if you might be desperate enough to force yourself to try a ffm again. I'm not comfortable suggesting this but maybe talk to your doctor about getting some temporary help to ease the anxiety the ffm produces. meds would add another variable, potentially confusing things, but if a ffm leaks less, how you feel could help determine whether or not your fatigue is related to sleep disordered breathing (it isn't worth much but intuitively I doubt it is).

I still think you should reduce your pressure, at least temporarily, to help with the leaks, to see what your flow rate looks like and to see whether and how much your events increase. fixed pressure to start to keep it simple if you choose to try it.

no guarantee, but vauto pressure support and some other setting options could help with flow limitations, leaks and comfort. I bought mine locally through one of those online buy/sell sites/apps (either craigslist or offerup I think) for $225 usd.

maybe others have other ideas?

RE: Please help with treatment review
sheepless thanks....

That's the worst leakage and night in some time. All I did was try new tape. I'll read up on real collars.

Would there be any benefit in using a real nasal pillows mask?

Still claustrophobic ... Dave

I'm lost....
I went back to bed.  didn't sleep much.
* no pad
* put tape back on

mask:  ResMed N20

Please help===what does this show?

(I played lots of football as a youth.  Large neck from football drills.  Broken nose severe times.  Checked out.  I had a bad fall at age4? landed with my head on a rock falling off a 10 foot retaining wall.  Broken so many bones I can't count them. separately.)

Is this positional apnea?  DaveL

RE: Please help with treatment review
sheepless I had tried a F30i. Horrible leaks. Couldn't keep it on....that was closest I came to wearing a full face mask. I kept it on for 2-1/2 hours. I really wanted it to work. Didn't. I didn't record when I tried it. I went through a 2 week period trying different masks...

RE: Please help with treatment review
Dave, I don't know what else to tell you. I've made some suggestions that you could try; beyond that I got nothin'.

trying things is the only way to know, no harm other than the expense, but if your leaks are mouth leaks, I don't know why a pillow mask would make a difference.

I don't think what you're seeing is positional because afaik chin tucking produces obstruction and the charts you posted are showing centrals.

looks like your current priority should be getting your leaks under control.

again, other than the leaks, your charts look pretty good. consider looking beyond apnea and pap for reasons for your fatigue.

Thanks sheepless. I don't know if I tried the right size F30i I've got a fit gauge. I'll try one again with the right size insert and headgear.

That will take some time to arrange. DME will loan me one (I think).

Overview...including leakage
This is the worst leakage I've seen in some time.

edit:  Found my old Dr Dakota "sleep well my friend" I stopped using it some time ago.  I'll use it tonight.  It's quite worn.  So I have an equivalent wedge to wear tonight.  Fingers are crossed

something interesting on another forum about apple watch & hearing....I'm interested in iWatch & OSA.

my iWatch 6 is a valuable tool. I want to figure out how to save and show screenshots here.  Shows movement at night with the accelerometer in the watch.

RE: Please help with treatment review
While you wait to get the mask to try, can you try
Min pressure 12 max 14.
EPR 1
I am thinking the leaking might start due to flex between inhale and exhale.

Also, can you make min and max on your flow line be -90 to 90? That will make close up flow snapshots easier to read and understand.

Let's get a 5 minute close up of a time you think you are asleep but the Iwatch says you are awake.

QAL

p.s. you should change the machine setting for mask type to the type you use. Some machines do different air delivery based on the mask type, and that may trigger mask leaks.
