When you enter REM sleep your muscles go into kind of a state of paralysis and your jaw relaxes. The first two large leaks you show were in roughly the time frames that REM would occur. You could be experiencing "jaw drop" and your jaw is dropping below the bottom of the mask causing leaks.

Hi David, thanks for your feedback. I suffer from TMJ disorder so I have problems with my jaw, and I do notice that towards the time I'm going to sleep my jaw does start relaxing and dropping down. What do you recommend in this case?

I would recommend what Opal already has. Either a soft cervical collar or mouth taping. Cover-Roll stretch tape is a good one...it's what I use. Although, if you go with the tape you 'could' go back to a nasal mask and you should be fine.

Hi, this is last nights data with the adjusted settings, I haven't recorded a night with the cervical collar yet, I will try that tonight.

Hi, please see last nights data, also please note that I wasn't actually asleep until 3.30am on the graph, just wondering if that shows a problem with my breathing in general and breathing pattern?
Tried to sleep wearing the cervical collar but couldn't as it wasn't that comfortable, I will try again tonight. Rest of the settings are the same as before.

So far, we have seen you are using fixed pressure at 10.4 at EPR between 1 and 3 results in an AHI of 3.4 to 4 events per hour, featuring mainly central or clear airway events. EPR does not seem to affect your results, so you can select that for whatever is most comfortable. I'd like to try reverse-titrating your pressure to a lower setting to see if that helps with these events. If you're game for something different, lets back your pressure off to 8.0 and use EPR 1 or 2 and see if that helps with the event rate and comfort. I don't know of a better way to get to an answer with your fixed pressure Elite than to try some alternatives and simply observe the results.

Thanks for your suggestion, I'm open to trying anything right now to get some good sleep. I'll try those settings tonight and report back tomorrow.

A couple questions...
Did you have a sleep study and did it show Central apnea? Have you tried 0 EPR? Keep pressure at 8cm. I'm not sure it would make a difference, but doesn't hurt to try.

Hi, yes I had a sleep study but it was around 3 years ago and I can't remember the results fully, but that was when the cpap was prescribed to me. I'll try the same pressure and reduce EPR to 0. I still need to get data from a night with the collar as well.
Also in regards to the pressure, when it's at 8 if I deliberately relax my muscles and put my chin down to simulate deep sleep I do get snoring, doesn't that mean the pressure is too low as it isn't enough to open my airway in that situation? |
