Starting Out - Guidance Please

RE: Starting Out - Guidance Please
Settings for ASVauto mode:
EPAP min: 5.0
EPAP max: 15.0
PS min: 3.0
PS Max: 15.0

These settings do not come from any divine intervention, but from Resmed recommendations.  The work starts after we see results.  You will have near zero events, but our next step is to dial in comfort.

RE: Starting Out - Guidance Please
First night of ResMed AirCurve 10 ASV attached
5/15.  I didn't sleep very well and my wife ended up leaving the room because it was loud (I was wearing ear plugs so not sure if it was more or less than Airsense 10).  I gave up at around 2:45am and then tossed and turned for a few hours then put it back on but I don't feel like I even fell back to sleep at 4:45am.  I'm fairly confident I had little to no sleep from 2:45am forward.

I have also attached a random 5 minute section at midnight incase there is anything useful in this.  If there is another time to look at please let me know, this was totally random section.

I feel about 3 out of 10 right now (10 being excellent)

I will continue using it but I'm as always open to comments or suggestions.  Thank you much!!

You have been through a lot to get to this point. I am positive that when some of the members familiar with ASV log in, They will be able to begin to assist you in timing and tuning to improve the comfort factor. Your AHI score 0.00, will be hard to improve upon.

Your wife left you because of the noise.  Dont-know   Was that noise from the machine or noise due to leaks from the mask and exhaust ports?

Did you check the hours report on the machine? Was it excessive?

Hang in there.

RE: Starting Out - Guidance Please
Well, you not only had near-zero events, you had zero events. We're going to reverse-titrate the PS max to calm thing down a bit, but it may take a few sessions. Use the same settings, but reduce PS max from 15, to 12.0. We may drop farther based on results. Based on the zoomed graph, I think we need PS min to be 4.0. Everything else, looks great other than the leak at midnight. So new settings are EPAP min 5.0, EPAP max 7.0, PS min 4.0, PS max 12.0

RE: Starting Out - Guidance Please
UnicornRider - My wife left the room 5/14 night because it was leaking a lot.  The machine has 3,000 hours on it and seems to be functioning properly.  I did wash my ResMed AirFit P10 Nasal Pillow and upon examination of it this morning it looks like the inside rubber got flipped over.  haha so that would explain why tonight, 5/15 it was sort of the same thing in terms of mask leakage and a bit loud.  She didn't leave me last night though (although this morning she did say it was annoying).  haha

Sleeprider - I changed the settings as suggested which is reflected in the 5/15 screen shot attached.
Min EPAP 5
Max EPAP 7
PS Min 4
PS Max 12

5/15 attached

I feel about 4 out of 10 this morning but as mentioned above upon further examination as to the constant fighting all night long about leaking it was found this morning that my P10 nasal mask wasn't working properly from recently washing it (a quick pull out of the 2nd inside liner with a tooth pick looked to spring it back to where it should be).  I'll try again tonight and hopefully that is the culprit with this latest ASV machine.  We'll see!  Thank you

RE: Starting Out - Guidance Please
I guess you have figured this out now, but the P10 nasal pillow is a pillow within a pillow. The only other thing is someone didn't realize that the pillow locking tabs are different widths, so you will only be able to connect it one way, or it will leak.

Feel 4 out of 10.  I feel beat up today.  I'm thinking the pressure is too much but I don't know;  any suggestions? I know the P10 pillow mask is resolved since I was not fiddling with it constantly like the night before but at the same time I also had a lot of large leaks. I do have a few other non-pillow masks I can try if the data isn't pointing to anything specific other than user error. Smile

RE: Starting Out - Guidance Please
Your event rate is zero. So the only way we're going to learn what works to take some drastic actions. If you think pressure is too high, let's try lower.

EPAP min: 5.0
EPAP max: 6.0
PS min: 4.0
PS max: 9.0.

RE: Starting Out - Guidance Please
Personal opinion I think the leaks are going to disrupt and maybe cause the ASV to hold higher PS than normal. I say specifically PS because EPAP range is pretty restricted.

RE: Starting Out - Guidance Please
Updated settings for the 5/17 evening to:
EPAP min: 5.0
EPAP max: 6.0
PS min: 4.0
PS max: 9.0.

5/17 attached

Much better with the new settings but I still feel pretty beat up.  I feel about 5 out of 10 right now.

It is a bit odd;  without CPAP/ASV machine I wake up fairly consistently between 1am and 2am completely wide awake and feel beat up.  With the previous Airsense 10 and it's settings that took weeks to tweak I stopped waking up "wide awake" around those hours, fell asleep quicker and felt more rested in the morning.  Although if you look at the history of the thread there were still some pretty rough nights where I felt 6 out of 10 or below.

Anything stand out?  This is all with P10 pillow mask.  I try to fall asleep on my back but if I don't after ~30 minutes then I turn to my side and try to sleep on my side.  I consistently feel more comfortable on my side or on my chest hence the P10 sort of fit that profile.

I absolutely know when I sleep like garbage and when I sleep well.  I can tell in my energy level, concentration levels, irritability, etc.  This last week has been rough!   Too-funny

Thank you as always in advance!
