Using a default trigger setting will likely increase your CAI. That is information to share with your doctor to help him connect the dots on why ASV is important. You do breathe with the stimulation of pressure support, but the machine has to be sensitive to your spontaneous efforts or respiratory rebound. ASV will do this much better than the Vauto. I agree with Dave. If you can tough it out a few more weeks, then your doctor should have what he needs to prescribe ASV. During this time, you should frequently share your discomfort and urgency to get ASV with your doctor, and perhaps even let him know that if he won't help, you can do it yourself. Devices are available at reasonable prices through DotMed, and you can demonstrate efficacy with that very quickly. My concern is that optimizing the Vauto could result in marginally acceptable results, misleading your doctor into thinking it is working. Feel free to put very-high trigger in if it just gets to be too much and you need a break.
