[Treatment] TheBinarySon's Treatment Thread
#21
RE: TheBinarySon's Treatment Thread
I'm at work and all I want to do is go home and sleep, or figure out why I can't seem to sleep. Anyway, question for you guys. Should I try using the Luna's "SmartC" and/or "SmartA" features? I'm not exactly sure what they do, the description in the manual (see image below) is vague, and a video I watched said something about the being more aggressive algorithms.  Thoughts?

You will be using APAP mode, so it will use Smart-A automatically.

RE: TheBinarySon's Treatment Thread
So Saturday I didn't sleep again so last night I was so exhausted I didn't want to switch over to APAP in case I didn't respond well to it.  The only thing I did last night was Turn on SmartC, which is a more aggressive algorithm that looks at the last 5 days of data or something.  Anyway, I slept a lot, and feel ok, in the sense that when I finally did wake up at first I kinda had energy and couldn't sit still which isn't normal for me.  But my body eventually petered out and I've been napping all day.  I'm going to try and stay awake now so I can sleep tonight with APAP as discussed.  That said, here is my data from last night.

Still don't understand the massive flow amplifications without there being any recorded respiratory events.

RE: TheBinarySon's Treatment Thread
By “flow amplification” do you mean the spikes or something else? If it is the spikes, we would need to see a 2-3 minute zoom into a specific instance. Most likely they are recovery breaths, possibly from the pressure being too low.

If you want to stay with fixed pressure, you can try slowly increasing it by small increments until the spikes settle down. You can also switch to APAP mode and let the machine titrate you within a given pressure range, something like 7-15, to see where the “sweet spot” of pressure is for you.

RE: TheBinarySon's Treatment Thread
Thanks for the post!  Yeah, I'm referring to the spikiness of my flow chart. I know a good flow chart is supposed to be relatively flat on top and bottom all night, so I'm assuming something is off. I'm gonna switch to APAP tonight with the range 8-13 (you suggested 8-15, Sleep rider suggested 8-11, so I'm gonna cut it right down the middle and do 13 for my max). I've already set it up on my machine so I don't have to fidget with it later. SmartA is set to on.

RE: TheBinarySon's Treatment Thread
Oh and I got a response from my Sleep Center to my question about the machines. The doctor forwarded my questions about Luna vs. Resmed to the Lead DME Tech at the center, who said:

"Good morning,

Your provider forwarded a message from you saying you wanted more information about the Luna G3 device. I was also made aware that you did some research on the ResMed and how they have app (3RD PARTY SOFTWARE) that you can view the data on.

Luna G3 also has an app. If you’d like to download them and see more detailed data you can.

Apple: https://apps.apple.com/us/app/react-heal...1039068820

Google Play: https://play.google.com/store/apps/detai...r&hl=en_US

The data available on your Luna Device is quite detailed as well, but if you want more detail, please download the app, and follow the instructions for it.

Machine pressure wise, both Luna G3 and ResMed device do the exact same thing. You have more pt controls with the Luna G3 device, but essentially, they run the prescription the same which is what is needed. Both devices have modems and can transmit data wherever there is cellular service.

If you have questions about your device, please feel free to message me back."

I didn't specifically mention OSCAR in my original email, but anyway I thought I'd share.

RE: TheBinarySon's Treatment Thread
It's your choice of course, but if it were me I would tell the DME that I am not satisfied with what they provided me and want to trade in the Luna for a ResMed Airsense 10 Autopap machine.  If they won't do the trade then tell them you want to return the Luna and will obtain the ResMed from another DME.  It's all money to them, but you need to be able to get what you want so that people can help you improve your therapy.  Folks here can help you dial in a ResMed to work better than a Luna.

RE: TheBinarySon's Treatment Thread
I would suggest you start using auto cpap mode initial setting
Min P - 8
Max p - 16
Reslex - 2
Sensitivity - 4-5 (important)
Try this setting for 3 nights then post charts

RE: TheBinarySon's Treatment Thread
Deborah K. - hi! Thanks so much for checking in again. I appreciate your concern, I really do, and it sucks that the Luna isn't compatible with Oscar yet, and we'll see if I have any major issues with it in the next week or so, but for now I actually like the machine.

Psychotech - thanks so much for looking at my thread! I did APAP for the first night last night with the exact settings you suggest except my Max P was 13, and while I haven't looked at the SD card data, my 95% pressure was only 9.5. I'll post my data in a couple hours, but I feel pretty good this morning (it helps that I've actually slept the last two nights), and I'm curious if at most I just need to move my CPAP pressure to 9.5 or 10.  But my Reslex was at 2 and sensitivity was at 4.  So hopefully that means I'm already close to what my pressure needs to be.

RE: TheBinarySon's Treatment Thread
Here's last night's data.  Again, I felt pretty good getting up this morning, but there's still a lot of spiky amplification in my flow chart.  Interesting to note my pressure never went above 9.5.  Should I give it a few more nights at the same pressure range (8-13), change it to 9.5-13, or go back to CPAP with 9.5?
