Talked to Supplier #2. They require a prescription for ASV. I didn’t talk to the respiratory therapist there. Should I have? I’m suppose to see the NP again next week. I know I’m gonna get push back. How and where can I get an ASV ASAP?

DotMed has many S9 VPAP Adapt machines from $340 to $700 and the higher prices have lower or zero hours. This machine is the predecessor to the Aircurve 10 ASV and has most of the same features including ASVauto mode, Easybreathe, and full data.

But will it work just as well as the ASV and will I need a prescription? Also, one of the contraindications for ASV is moderate to severe predominate CSA. Isn't that what I have?

It is ASV, but the price is half the current Aircurve 10. Therapy algorithms are the same. I have never seen a prescription request made of any member buying from this source. The major differences are form factor, and a lower data storage capacity. Also you need to lock the SD card when uploading data to OSCAR because the S9 will reformat cards with hidden index files from Windows.
What the S9 lets you do is demonstrate the efficacy and medical necessity of ASVto your doctor allowing him to have a “lightbulb” moment and prescribe ASV without further testing.

The RT at supplier 2 may be able to assist with a prescription. Or go the Dotmed route no prescription required there

No, it is contraindicated in severe Congestive Heart Failure (CHF) patients with left ventricular ejection fraction less than 45% and Cheyne-Stokes Respiration (CSR). Congestive Heart Failure aka CHF is the only contraindication to ASV I know of. What heart condition(s) has any of your doctors told you that you've got? When I tested for the ASV via the currently still required echocardiogram, I showed a 55% LVEF / Left Ventricle Ejection Fraction. That's 10 above the minimum 45%. And my heart isn't great, with this other issue going on called PVC that messes up the heart rate at times. It causes palpitations for me. But for me, ASV is the least PAP I'll accept. I may go to NIV, but I won't go less than that or ASV. Those jokers aren't torturing me again on CPAP/BPAP.

Resmed marks all periodic breathing as CSR. Not the same thing. Compare the pattern of what is flagged as CSR with the chart I posted above. CSR has a very distinct rhythm with a central apnea at the nadir of respiration and is a classic flag for CHF. Periodic breathing occurs for many reasons and is basically a variable flow rate. The diagnosis of CSR is serious...you don't have it.
