RE: VCOM & Bi-Level
Merry Christmas!
Update on my VCOM 'Testing'.
I am actually using VCOM all the time now, no reason to take out really, it is invisible in a sense.
I have tried it with every mask i have, pillows, nasal, full face and now with new REsmed air sense 11 machine.
I have parsed the data up and down; hard to find definitive conclusion, until this....
Respiratory Rate!
No more hi peaks for 95% and 99.5% , as per Overview Tab in Oscar.
Very stable.
Hoping you all that are testing will look at your Overview and check for that?
It makes total sense too...
VCOM reduces Peak Inspiratory Flows, like when you take those large breaths for whatever reason ( CO2 and air hunger come to mind for me)
Looking forward to hearing what you are finding with your results on this!?
And one other thing, small point maybe, but to clear up.
I hear a lot of people describing VCOM, one way or another, as changing Expiratory pressure.
It does NOT change it!
Dr. Noah says hardly at all which is fair enough.
What it does is slow down the flow during INHALE, then when you EXHALE the *PAP machine has a chance to rebuild pressure and that is the EXHALE PRESSURE.
So, INHALE pressure is reduced proportional to size of breath and then EXHALE pressure is whatever your machine is putting out at that time.

RE: VCOM & Bi-Level
Thanks for that update and linking VCOM to respiratory rate. I've nearly finished my trials with EERS and my data is looking good except during REM. That's good insight that the change in inspiratory flow is proportional to the breath. Looks like my next step will be adding VCOM back in with the EERS.
EERS = increase Tidal volume
VCOM = stabilize respiratory rate

did u check your Overview Tab?
Comparing before and after VCOM and Respiratory Rate Chart?

@Seepak
Yes, I put the VCOM back in last night. My 99.5% resp rate went from 25-27 down to 18. With the combination of VCOM and EERS I can tolerate much higher pressures. I will experiment for a few days and post a chart here and on my EERS thread.

great to hear!
and your comment, "With the combination of VCOM and EERS I can tolerate much higher pressures." just hit me about my recent therapy!
short story
New Resmed Air sense 11, Auto, 7.4 cm to 10 cm , Dreamware FFM , VCOM @ mask hose end.
I have NEVER even thought/had/wanted higher than 8 cm pressure in the past.
And i attributed high CA events to the higher pressure ( now know this is not true)
And NEVER had an APAP, only CPAP.
Hmmm, things are getting interesting!?
