newly diagnosed, 3 weeks cpap. Help!
Hi Everyone, I've been recently diagnosed with sleep Apnea in the last 2 months, I had an AHI of 104 with oxygen levels falling below 70 percent and an average of 82 percent. I've been diagnosed by the NHS with a home study. I've been given a Phillips dreamstation and they recommended the Resmed Airfit F20 full face mask. I've been using the CPAP for 3 weeks tomorrow and things have been going generally ok, I'm managing to keep the mask on but have been having some leaks around the eyes and I've been given a Resmed Airfit f30 to try too. The last week my AHI has been below 5 which I'm quite happy about and generally I've been feeling a million times better than I have been which I'm really happy about. I've downloaded Oscar and uploaded my data which i don't really understand. What I'm hoping is some help to improve and fine tune, lowering my AHI consistently. I've also been having aerophagy pretty bad and I'm extremely bloated every morning which can be painful, I'm hoping I can resolve this issue too if anyone has any advice. Thank you to anyone who can point me in the right direction to hopefully resolve some of these issues.

One issue you'll need to be aware of is "Positional Apnea". Look at the Obstructives and
Hypopnea where they are clumped together. You are sleeping in a position that is cutting off your air. This is usually from "chin tucking", where your head falls forward into your chest. Some things that cause this are back sleeping and using more than one pillow or a too high of a wedge type pillow that will push your head forward. This can also happen while on your side. Just be aware. Try to use just one thinner pillow if sleeping on your back. I think you can clear up a lot of your apneas if you can master this problem. Read through this article and you will see some similarities. Although the examples are much more extreme than what your chart shows. https://www.apneaboard.com/wiki/index.ph...onal_Apnea One other thing, your minimum pressure is a little too low. If you are able, move the minimum pressure so it's closer to your median pressure. So try minimum 12cm. Also, try to wean off using the ramp feature. If you feel you need it, then set it at a higher pressure than 4. What is your Flex setting? Summary: Minimum 12cm, Maximum (leave at 20 for now) Set Flex at 2cm. Set Ramp to Off or try 8cm. Report back after a few days.

With leaks that shoot air into the eyes, I think you’re on the right track to trial an F30. Could you mention some of your mask issues with that one? Your leaks need addressing, as you may already have guessed.
LankyLefty on YouTube has a video of just about every mask and what some of the idiosyncrasies are, as well as how to adjust it (every mask is finicky in a different way, alas). |
