That is how I'd see these. We call this sleep/wake junk or SWJ. As I understand it, there's a transition between sleeping and waking breath control and the possibility of tossing and turning to gain a comfortable position are thrown in to the mix ax as the cause of these events.

Some of it might be SWJ but during the titration study the apnea were still going on even once he was supposedly in N3 sleep which was one thing that surprised me. During the titration study his PS was higher so centrals may have been worse though.
With the Vauto machine you have pretty limited ability to try and treat these apnea. About all you can do is try a higher EPAP, half these apneas last night were flagged as obstructive so it may help. Otherwise you only have two other options imo. 1) Ignore these apnea. They mainly appear during short periods during/after sleep transition and may not have that significant of an effect on sleep quality. 2) Step back up to a machine that can treat central apneas. ASV, ST or ST-A.

Higher PS would make them worse of course? The Vauto has really helped my RR so will stay with it for sure.

No. Higher EPAP is not likely to resolve centrals. You have found a solution with bilevel therapy that works to resolve the irregular breathing, flow limits and obstructive events. Targeting sleep transition centrals is not worth chasing at this level in my opinion, and if you really look at the events, they are occurring when your EPAP is higher or at tmes when we wold normally consider it SWJ. I think these are nothing to worry about at this time, and you should simply focus on comfort and the success you have apparently found.

I'm not going to suggest changes to chase events that are of the pattern of SWJ. Do not toss comfort out the window for this.

So I noticed something interesting in my data that made me remember this thread.
I had a short section of the same uneven breathing Mal has/had regularly. I don't have a pacemaker so pacemaker does not appear to be the cause as we had theorized. I do have regular cardiogenic oscillations but this was the first and pretty much only time I have seen this kind of fluctuation. Anyways just figured I would share this as something to note, hopefully things are still going well as they sounded like you had found some improvement. |
