BiLevel Settings From CPAP
I’m looking for a little guidance in trying to set up a Resmed Aircurve. I’m currently on CPAP with a pressure of 13.2 and a EPR of 3. Any advice where to start? I’m very easy to wake up, don’t feel rested, and after having my Oscar reviewed they thought Bilevel might help. Any help will be greatly appreciated.

If you will post a couple of recent Oscar reports folks here will be better able to advise you as to settings to try.

Please see attached. I included a few days one with pressure set at 13.2 with a EPR at 2, next day 13.2 with EPR at 3, and 14 and EPR 3. Let me know your thoughts, Thank you

I do wake up alot during the night. I was told by a Oscar enthusiast that Im waking up because my breaths are spiking not smooth and most of the time not registering an event. This is why I went to a EPR of 3, and am trying a Bi-Level. Maybe this will help me sleep more through the night and be easier to breath. Im new to this and trying to learn.

This was my first day with the on BiPap. Feel ok, not sure if I should change any of the below numbers? Some time it felt like it stop my in-breath from the full breath, right at the top of the breath. My lungs feel like they got a little bit of a workout. Any suggestions or insight would be greatly appreciated.

EPAP 6.8
PS 5.2
Ti Max 2.0
Ti Min .3
Triger & Cycle Very High

Your CPAP settings would have been IPAP 13.2, EPAP 10.2, PS 3.0. The Chart you posted is showing an Airsense 11 and CPAP settings, but the charts indicate I think you're being too aggressive on PS with the pressure 12.0/8.0 at PS 4.0, however the 95% and maximum stats suggest more PS. I'm confused.

If you are starting out using a Vauto, clean up your card and start a new Oscar profile. Initial settings should start at EPAP min 8.0, IPAP max 15.0, PS 4.0. Keep the defaults for TiMin 0.3, TiMax 2.0, Trigger and Cycle Medium. I never suggest a change to higher cycle sensitivity in healthy individuals, and higher trigger can be used is you observe CA events, but let's not start at Very High.

Thank you for the reply. Day 2 Bi-level I took your recommendation for the settings. Looks like I had more episodes, breathing felt fine and lungs feel fine this morning. What do you suggest to get rid of those  Hypopneas?

Change trigger to high and the events of this type should be reduced. Everything else looks god and you seem to be in a pretty good situation for pressure range. You have some mask leaks that appear to be the seal from the mask moving around. These should not affect therapy, but may be disruptive. I wanted to see this baseline with default settings for trigger, cycle and Ti so we could narrow down what was needed.

Ok, I changed to the advised settings. AHI was down, but I dont feel rested and I woke up at least 6-8 times, but fell back to sleep right away. Maybe I feel like im not getting enough oxygen. If it helps Im a elite pro athlete, so Im really know my body well. Any sugestions would be greatly appreciated, and thank you for all your help and support to getting me well!
