Lowenstein Prisma 20a - How can I get acclimated?
I have been using an airsense 11 a few years. It was quite a struggle at first but I’ve gotten my pressures dialed in and everything was ok. But I was still tired most mornings.

I researched and got a Lowenstein 20a machine because I thought the early IPAP pressure would be very good for me since my Oscar waveforms looked flat on top.

After waiting 3 weeks for it, I tried it last night. I closely mirrored my settings and it seemed comfortable. Well I was trying to sleep for 2 hours and didn’t realize it was because of the motor ramping up and down sound. Now I can’t hear this without my earplugs in but I need them in. The airsense 11 I could hear my breathing so that kind of put me to sleep. Now all I hear is the motor ramping up and down. 

I need the epap support so this machine sounded perfect in that it gives me the epap support but then is right there for iPap.

What can I do? I just spent $1600 on this but after fighting it I had to use my 11 again and only slept 5 hrs and lost 3 hrs of my normal sleep. Please help thank you.

I am also a new user of the Prisma 20A with only two nights in.  You describe, "the motor ramping up and down sound." I hear some low sound for perhaps the first five or so minutes and then it goes silent. I have Soft Start off, so I know the machine in not ramping. I don't know why that early sound occurs but I'm sure it serves a purpose. I also learned that the display will not go dark if you do not log out of the clinician setting. When logged out the display goes dark after 30 seconds. I think this will take a week or two to really get to understand the workings of this machine. Biggest benefit I see so far is my 90% pressure dropping from 12.5 to 11. Also, the inhalation and exhalation seem so much smoother than my ResMed AS10. And after the early detectable I described it goes absolutely quiet for me.

I failed to mention that I did better without the "Dynamic Mode" turned on. Also, if you were in Soft Pap3 I would reduce to Soft Pap 2. That little burst of extra pressure on inhale could be causing the ramp like sounds you are hearing. Worth a try.

Yes I did try to change to 2 it still had it. I was going to try dynamic mode next I heard a lot had success with that

I have been using my brand new Lowenstein Prisma20A for 2 weeks and I am still fiddling with my setup.
Prior to this I used a Philips DreamStation for 8 years and eventually switched it from APAP to CPAP with immediate improvement.
Be sure that you hold the "i" button down for 4 seconds and log in as a clinitian. You will eventually find your sweet spot.

Yep Dyn mode works for me.

I didn't know the Prisma20A had a doctor mode.

Not that it paints the total picture by any means, but my AHI last night was about as good as it has ever been at 0.28. Slept for over seven hours for the first time in a long time. Don't know exactly how to explain it but the change from exhalation to inhalation is the quickest and smoothest I have ever experienced with any machines. I agree with BB37 to continue to fiddle with you clinician setup and, "You will eventually find your sweet spot."

Yeah I’ve been messing around the settings. I think my main issue is the machine forcing me to inhale and I’m not ready for it yet. My sirsense 11 did not do that I could breathe my own rhythm. Which is hard and I can’t fall asleep using the lowenstein. 

Would a bipap help me? I know I need at least a 10 inhale but would love to exhale to 5 all the time? How does that work exactly? I know lowenstein it’s very nice but that forceable inhale I just can’t do it

I don't have what I could consider a "forcible inhale." For me it's just a very smooth transition from exhale to inhale. I think it must be something within your settings. I'd go on the seller's website about 1pm your time and chat with Rob or Jason and see what advice they may have. They are very helpful and if they don't have your answers, I'm sure they can put you in touch with an English speaker with Lowenthal in Germany. Lowenthal's customer service is supposed to be excellent.
