New to all of this… confused which machine to get

(01-10-2025, 05:33 PM)Sleeprider Wrote: Get the machine, erase all data in the settings.  Configure for the correct mask (full-face) and start with these settings:
Mode: Vauto
EPAP min: 5.0
EPAP max 10.0
PS: 3.0
Trigger Sensitivity: Very-High
(default Ti-min 0.30, Timax 2.0, cycle: medium).

Sleep as best you can and post a chart. We will go from there.

Finally got my machine and had my first night last night. Was an interesting experience... had initial worries after the first couple hours of waking up wouldn't be able to adapt and handle it... woke up around 1am and felt like I was being suffocated and ripped the mask off in a panic. Left it off for about 30 minutes until I settled down enough to try again. After that, I was able to successfully sleep pretty much through the rest of the night.

MyAir app reported 71/100 for whatever that's worth. OSCAR chart attached.

A huge THANK YOU in advance for the help!

Well that's just awful! Leaks are out of control so your mask does not fit. I think you're going to have to find something else. Mask Primer
You have huge clusters of obstructive events which are likely positional. Positional Apnea
We need to increase pressure. Let's reset to mode Vauto, EPAP min 8.0 Max IPAP 16.0, PS 4.

From your chart, all the events correspond to periods of high leaks, so your mask is the highest priority to get right.

Tried the new settings and also spent some time adjusting/tightening the mask. Had what felt like a pretty good sleep last night and here's my results.

That's a much better result, especially after 01:30. Before that is a cluster of positional apnea. Whatever you did for leaks really helped. Anytime we can cut events and leaks to this extent we can call that a win and try to build on it by doing it again.

Edit

It does look like AHI was cut from 25 or so to about 7, so a decent improvement. Keep working at it.

Do I need to make any adjustments to the machine settings? Or do we feel we have a decent baseline for the time being?

I will experiment more with my mask to see if I can get a better fit.

Let's see if the improvement sticks or if you can make some progress with the positional apnea. We try not to move through setting changes too fast. Focus on the fact you had 6-hours of very good therapy results with less than 1-AHI, and all the events actually happened in the first 1.5 hours. The settings are probably okay, but I would not be surprised if we go to a maximum pressure of 18 to clean it up. I think the really bad start is due to your very low Ramp starting pressure. If you can turn that off, we would be way ahead of the game rather than falling behind from the start.
